Description
NOTICE OF INTENT TO SOLICIT SOLE SOURCE The Defense Logistics Agency, Troop Support, Philadelphia, PA Medical Directorate intends to award a Firm Fixed Price, Sole Source contract under the authority of FAR 13.106-1(b) to Johnson and Johnson Vision (formerly known as) AMO Sales and Service, Inc. 1700 E. St. Andrew Place Santa Ana, CA, 92705 for the procurement of the following brand name treatment and calibration key cards: Product Number: 00400-7000 Description: 10TX AddedVue Pak, ST 23 DOMESTIC Quantity - 50 Product number: 1100-0100 Description: CustomVue (R) License Fee Quantity - 500 Product number: 1100-0201 Description: AddedVue Enhancement License Fee Quantity - 100 This requirement is for the purchase of treatment and calibration key cards used in support of the Femtosecond and Excimer Ophthalmic Laser systems at Irwin Army Community Hospital, Ft. Riley, KS. In accordance with FAR 13.106-1(b), only one source, Johnson and Johnson Vision, formerly known as AMO Sales and Service, Inc. (AMO), is capable of providing the supplies required at the level of quality required because the supplies are unique or highly specialized. The treatment and calibration key cards are essential for use with the lasers and can be used with either laser. Each card can be used ten times and they act as a key (permission) to operate the equipment to do a procedure. Due to their proprietary nature, only the original equipment manufacturer (OEM) providing the hardware can provide the treatment and calibration key cards. Delivery schedule will be discussed prior to issuing the delivery order. The anticipated award date is August 17, 2018. This notice is not a request for competitive quotes; however, all responsible sources and interested parties may identify their interest and capability to meet the requirements by submitting technical data for this requirement within 5 calendar days of this notice posting (no later than August 14, 2018), by 11:00 AM EST, to Paula Wolstenholme, via email at paula.wolstenholme@dla.mil, which shall be considered by the agency. DLA Troop Support intends to solicit this requirement in accordance with FAR 13.106-1(b)(1)(i), thus no written solicitation will be issued.
