TUSCALOOSA VA Medical Center NCO 07 Contracting 3701 Loop Road East Tuscaloosa, AL 35404 Subject: Solicitation 36C24718Q9440, Bowling Alley Maintenance- Request for Quote Dear Prospective Offerors: The VA Medical Center invites your firm to submit a quote for the subject requirement as indicated in the Price/Cost Schedule of the solicitation. The Government intends to make an award of a firm-fixed price contract for this procurement. Please submit a quote in accordance with the following instructions: Complete blocks 12, 17a, 30(a) (b) and (c) of the attached SF1449. In doing so, the offeror accedes to the contract terms and conditions as written in the Solicitation. The Solicitation constitutes the model purchase contract. Complete Contractor information on page 3 of the solicitation Complete PRICE/COST SCHEDULE starting on page 7 of the solicitation Submit a quote on company letterhead Your reply to this request for quote must set forth full, accurate, and complete information as required by this solicitation.
