Description
1 DESCRIPTION/SPECIFICATIONS/WORK STATEMENT Parking Lot Lighting Service and Maintenance Newington and West Haven, CT REV: 20180720 Abbreviations: CO -Contracting Officer COR-Contracting Officer's Representative JCAHO -Joint Commission on the Accreditation of Healthcare Organizations VA -VA Connecticut Healthcare System VACO -Veterans Affairs Central Office VACHS- Veterans Affairs Health Care System 1. CONTRACTOR: Contractor shall provide all labor, equipment, material and transportation of the normal service requirements for the Government-owned parking lot and other exterior lighting detailed below, located at the VA Connecticut Healthcare Systems West Haven and Newington Campuses. A. Period of Services: The requirement is for a base 12-month period of performance with four (4) 12-month option periods to extend. The review and repairs of lighting may be required in the evening hours while the lights are on, or as needed by the VA. Required services shall be provided as needed, Monday through Friday, Federal Holidays and weekends except during periods described elsewhere in this description. B. Condition and Age of Equipment: All equipment shall be in good working order at the start of the period of performance. The equipment may be examined by potential contractors prior to quote submission. The Contractor shall also observe the temperature, relative humidity, vibration, input voltages, fusing, circuit breakers, and all other aspects of the installation as presently made. C. Parts: 1. Only new or like-new standard parts shall be furnished by the Contractor. All parts shall be of current manufacture and shall have the versatility of the presently installed equipment. 2. All newly-installed replacement parts become the property of the Government. Replaced parts are to be disposed of by the Contractor after obtaining COR approval. 3. Any worn or defective part shall be replaced free of any charges. Operating and consumable supplies are not part of this contract. 4. When parts are required and are not covered by the preventive maintenance inspection and any intervening services, such needs shall be reported to the COR along with a charge estimate. The COR will evaluate these needs and authorize appropriate action. These parts shall be installed under the terms of the contract. 5 D. Performance: 1. STANDARD: All equipment shall be kept in proper operating condition and shall operate as intended. 2. SERVICE CALLS: The Contractor shall respond to service calls within two (2) hours, and all work shall commence within 48 hours after receipt of telephonic, written, or verbal notification. The work shall proceed to completion without delay. a. The contractor shall provide an emergency contact number with a responsibility to respond in case of an emergency, whether during normal business hours, evenings, and/or weekends. b. Contractor shall provide the VA with a statement describing an acceptable arrangement for notifying the Electrical Shop when equipment should be made available for servicing. 3. QUALIFICATIONS a. Any contractor personnel performing services under the resulting contract must be licensed, certified, and/or trained, as applicable, to perform such services. b. All work performed shall be accomplished in accordance with manufacturer's instructions, including but not limited to adjustment, calibration, cleaning, lubrication, testing, disassembly, checkout, replacement of worn or defective parts, etc. c. The Contractor s staff shall include an Electrical Trade person(s) licensed within the State of Connecticut to perform the tasks required; The Tradesperson shall have the skills and training to perform all aspects of work required according to the NFPA NEC and NFPA 70E. d. Contractor equipment used in the performance of this contract must be certified for such use via inspection, certification, etc., as applicable. Contractor personnel using the equipment must also have the required training and/or licensing for the equipment. e. The Contractor shall submit all licenses and training certificates upon contract award for the staff that will be working at the VACHS Campuses. 4. INVENTORIES: Contractor shall be responsible for a twice-monthly inventory of all exterior lighting at both the West Haven and Newington campuses, or as needed. a. WEST HAVEN: The inventories shall occur during the 1st week and 3rd week of the month; b. NEWINGTON: inventories shall occur during the 2nd and 4th week of the month. 5. DISPOSAL a. Contractor shall be responsible for proper disposal off all used lighting material. All applicable material shall be properly recycled in accordance with CT DEEP Non-hazardous waste requirements. Recycling logs shall be provided to the COR. All metal, including ballasts, must be recycled. 6. CATALOG a. The current luminaries do not have sufficient identification. The contractor shall identify and catalog the luminaries during the course of the base year. E. Authorized Service: 5 Only those services specified within this contract are authorized under this contract. Before performing any service or repairs of a non-contract nature, the CO shall be advised of the reasons for this additional work. If appropriate, the CO or his/her designee may authorize the additional services or repairs under a separate purchase authorization. The Contractor is cautioned that only the CO or his/her designee may authorize additional services or repairs and that reimbursement will not be made unless this prior authorization is obtained. Call-back service within 90 days for the same type malfunction shall be at no charge to the Government. The Contractor shall provide all service and technical manuals necessary to maintain the equipment in this contract. Engineering updates to the equipment must be provided at no cost to the Government so as to maintain the system in its optimum condition. F. Reconditioning: Reconditioning and/or extensive repair work determined by the CO not to be economically feasible is not included in this contract. The Government reserves the right to obtain such service from other than the listed Contractor. G. Reporting: 1. CHECK-IN: For repairs or services performed during normal working hours, the Contractor's repair person shall report upon arrival to the FMS Engineering Section, The Contractor's repair person shall report in person to the Electrical Shop B-15. a. NEWINGTON: During normal business hours, contractor shall check in with Daniel Smith or his designee. Outside normal business hours, contractor shall report to the Boiler Plant. b. WEST HAVEN: During normal business hours, contractor shall check in with Todd Spangler or his designee. Outside normal business hours, contractor shall report to the Boiler Plant 2. CHECK-OUT: The Contractor's repair person shall submit in writing a complete report of any services or repairs performed for each item of equipment, and shall also include a list of replacement parts when applicable. The report shall also include any Contractor recommendations necessary to maintain the equipment in operating condition. This report is required regardless of the outcome of each service call and shall be signed by the monitoring COR or the supervisor of the Electric Shop. 3. For repairs or services required during other-than normal working hours, the Contractor's repair person shall notify the Boiler Plant Operator prior to effecting repairs, and upon completion. The repair person is still required to complete a Report of Service Rendered. 4. The equipment serviced must be clearly identified by name, serial number, and VA property number. 5. The Contractor shall provide the VA with reports which describe the maintenance and repairs services performed on the equipment in sufficient detail as to be acceptable to the field inspectors of JCAHO. 6. Both the check-in and check-out procedures defined above are mandatory and strictly enforced. 7. Payment of invoices may be delayed if the appropriate reports are not completed properly and are not submitted to the Electric Shop as required above. 2. CONTRACTING OFFICER: The Contracting Officer administers the contract to ensure that the contractor performs in accordance with contract requirements. The CO is the only responsible official for the Government in all contractual matters that involve change, direction, work performance, time, and money. All correspondence and any questions will be directed to the CO for advisement. 5 3. FEDERAL HOLIDAYS: The ten Federally-observed holidays include: New Year s Day Birthday of Martin Luther King, Jr. Washington s Birthday Memorial Day Independence Day Labor Day Columbus Day Veterans Day Thanksgiving Day Christmas Day 4. IDENTIFICATION, PARKING, SMOKING, AND VACHS REGULATIONS: IDENTIFICATION: The Contractor's personnel shall wear visible identification at all times while on the premises of VACHS. PARKING: It is the responsibility of the Contractor to park in the designated parking areas. Information on parking is available from VACHS Police Section. VACHS will not validate or make reimbursement for parking violations of the Contractor under any conditions. SMOKING: Smoking is prohibited inside any buildings at VACHS and within 50 feet outside of any building. Possession of weapons is prohibited. Enclosed containers, including tool kits, will be subject to search. Violations of VACHS regulations may result in citation answerable in the United States (Federal) District Court. 5. CONDUCT: a. All contractor personnel are subject to the same rule of conduct and standards as Government employees. b. The speed limit on VACHS installations is 15 m.p.h. Citations for violations of speed limits, parking violations, or other infractions will be issued by VACHS Police. These infractions are Federal offenses which must be handled in a Federal District Court. c. No alcoholic beverages or drugs are allowed on the installation. VACHS has implemented a smoke free environment policy. There are authorized smoking areas outdoors. 6. INVENTORY a. Additional Notes: 120/208VAC% 277/480VAC is the applicable voltages for the lighting. Any items requiring service shall be repaired immediately or within the 48-hour window. Exceptions will be that if there are damaged poles and/or fixtures that are not normal stock items. The VA will supply the contractor with a site map of the facility in question and identify any light fixtures requiring repairs. The contractor shall keep a log of repairs made to light fixtures. A Microsoft Excel spread sheet of the repair log shall be required quarterly. Contractor may be required to work nights and/or weekends to repair, install, or replace lights and fixtures in parking lots that are usually occupied during normal working hours.
