Solicitation Number: 36C25818Q9766 Solicitation Type: Request for Quotation (RFQ) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-99-1 / 07-16-2018. This acquisition is 100% set-aside for Service Disabled Veteran Owned Small Business (SDVOSB) Concerns that have been verified by the U.S. Department of Veterans Affairs Center for Veterans Enterprise (CVE) in accordance with 38 CFR § 74 VA Veteran-Owned Small Business Verification Guidelines and appear in the Vendor Information Pages (VIP) database found at http://www.vip.vetbiz.gov/. The North American Industry Classification System (NAICS) Code associated with this procurement is 339113 Surgical Appliance and Supplies Manufactures and the small business size standard is 750 employees. The Contract Line Item Numbers (CLINS), items, quantities and units of measure requested are: CLIN ITEM QUANTITIY UNIT OF MEASURE 0001 MIDMARK 626-001 BARRIER-FREE POWER EXAM TABLE W/RECEPTACLE, PELVIC TILT, DRAWER HEATER 9 EA 0002 UPHOLSTERY TOP 28 WIDE PREMIUM, SHADOW 002-2000-232 9 EA 0003 RITTER 244 -001 BARRIER-FREE BARIATRIC POWER TREATMENT TABLE 1 EA 0004 UPHOLSTERY TOP SHADOW 002-0861-232 1 EA 0005 Inside delivery and setup at end user location, debris removal. Awarded vendor to coordinate delivery with the facility POC prior to shipment. Tables are going into an area that is undergoing renovation. Vendor will need to coordinate manufacture and delivery of tables to coincide with availability of end use location due to fluctuation in renovation timelines. 1 JB All items shall be shipped Free on Board (FOB) Destination and delivered on or before September 30, 2018 to the following location: Southern Arizona VA Health Care System Attention: Building 80 3601 S 6th Ave Tucson AZ 85723-Q001 Please mark all shipments to the attention of Robert Klein, Administrative Officer in Primary Care, at the Southern Arizona VA Health Care System. The provision at FAR 52.212-1, Instructions to Offerors Commercial Items applies to this acquisition. Addenda are attached to the provision. The provision at FAR 52.212-2, Evaluation Commercial Items does not apply to this acquisition. This acquisition will be evaluated based on price. This acquisition will be awarded to the lowest priced responsive and responsible offeror on an All or None basis, meaning that the offeror must be able to supply the total quantity requested of the acquisition to be considered for award. All offerors are advised to include a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications Commercial Items with their quote. The clause at FAR 52.212-4, Contract Terms and Conditions Commercial Items applies to this acquisition. Addenda are attached to the clause. The clause at FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders Commercial items applies to this acquisition.
