- A.09.03_W81K0218Q0177_Att1 The Regional Health Contracting Office - Pacific, Bldg 339 Clark Road, Ft Shafter, HI 96858, intends to solicit, negotiate, and award a Firm Fixed Priced Contract for Various medical gases for Tripler AMC Department of Medicine. This is a request for quotation (RFQ). This solicitation number is W81K0218Q0177. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-99. The Standard Industrial Classification Code is 6515, size standard is 1000 employees and the NAICS code of 325120. This procurement is 100% SB Set-Aside. Provision at 52.212-1, Instructions to Offerors -- Commercial, and its addendum applies to this acquisition and a statement regarding any addenda to the provision. See Att1. Evaluation Shall be conducted in accordance with the procedures of section -- Commercial Item Evaluation Factors for Award. See Att1. Offerors shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items, with its offer. The clause at 52.212-4, Contract Terms and Conditions -- Commercial Items, and its addendum applies to this acquisition. See Att1. The deviation to clause at 52.212-5, Contract Terms and Conditions Required To Implement Statutes Or Executive Orders -- Commercial Items, applies to this acquisition. See Att1 for the FAR clauses that are applicable to this acquisition. The Defense Priorities and Allocations System (DPAS) and assigned rating is not applicable. The Government intends to award a Firm Fixed Priced contract for a base period of 01 Oct 2018 through September 30, 2019 with one (1) one-year options to the successful quoter.
