- Clauses and provisions Plans at front gate Statement of Work Example This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is W912TF-18-T-5040, and is issued as a Request for Quote (RFQ), unless otherwise indicated herein. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-95. The associated North American Industrial Classification System (NAICS) code for this procurement is 238990- All other specialty trades (Size Standard of $15 Million)., a small business size standard of $15.0M. This requirement is 100% set-aside for small business, and only qualified vendors may submit quotes. The solicitation will start on the date this solicitation is posted and will end on 7SEP2018 at 1PM Time Eastern Time. FOB Destination shall be 302 Newmarket Street, Newington, NH 03803. This requirement is for a Minuteman Statue. The Government contemplates award of a Firm-Fixed Price (FFP) contract. Delivery Date: 90 Days After Date of Contract Supply The NH National Guard requires the following items, Brand Name or Equal, Meet or Exceed, to the following: CLIN 0001: Large Art Company 6FT Minuteman statue Life size (6 ft tall) Hand crafted. Produce, transport, erect and install a 6' tall, by 4' wide, sturdy, bronze Concord Minuteman statue. Statue will be made to withstand summer and winter weather typical of New Hampshire and sturdy enough to ensure it can hold a reasonable person's weight hanging off the musket without breaking. Statute will be new and not reconditioned or refurbished. (See Concord Minuteman Picture) CLIN 0002: Using approved industry standards, properly mount and install bronze statue on top of 4' tall, by 4' wide woodbury gray granite stone base/slab with thermal top and edges with 6" chamfer at top edge (the chamfer is a 45 deg cut 6" down from the top edge) CLIN 0003 - Provide and install 4" aluminum base ring around statute base and fill with 12"-3/4" crushed red stone compacted in 6' lifts CLIN 0004 - Installation of all required items IAW attached SOW Site Visit: The Government will hold a site visit at 302 Newmarket Street, Pease ANGB, Newington NH 03803 on 28AUG2018 beginning at 9:00 am Eastern Standard Time (EST). This is not required but strongly encouraged. All interested firms may attend with a limit of two (2) attendees per firm. Each individual must bring a Government issued identification in order to enter the Government installation. Please submit, in writing, the names of attendees to Isao Hattori at isao.hattori.mil@mail.mil by 2:00 pm EST on 27AUG2018. All technical or contractual questions shall be submitted in writing via e-mail no later than 11:00 am EST on 5SEP2018 to Daniel Ash Daniel.p.ash.mil@mail.mil. Telephone inquiries will not be accepted. If the Government responses to technical questions affect the any other portion of the solicitation, then an Amendment of Solicitation will be issued. The subject of your email must include your company's legal name, state the nature of your email and include the Solicitation Number W912TF-18-T-5040. For example, the subject of your email should be similar to the following: Quote Submission by Company XXXXX, Solicitation Number W912TF-XX-X-XXXX Company XXXXX Question Regarding Solicitation Number W912TF-XX-X-XXXX The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The basis of award is LPTA (Lowest Price Technically Acceptable). LPTA for this requirement is defined as an offer that proposes the lowest price for technical acceptability in meeting or exceeding the required specifications as detailed within the SOW or the solicitation. Quote must be good for 45 calendar days after close of the solicitation. Shipping must be free on board (FOB) destination CONUS (Continental U.S.), which means that the seller must deliver the goods on its conveyance at the destination specified by the buyer, and the seller is responsible for the cost of shipping and risk of loss prior to actual delivery at the specified destination. This solicitation requires registration with the System for Award Management (SAM) in order to be considered for award, pursuant to applicable regulations and guidelines. Registration information can be found at www.sam.gov. Registration must be "ACTIVE" at the time of award. The selected offeror must comply with the following provisions and clauses.
