- This is a SOURCES SOUGHT/MARKET RESEARCH only, pertaining to brand name supplies and equipment for the VAMC Detroit, located at 4646 John R Street Detroit, MI 48201-1916. If a solicitation is published, one would be released in proximity to August 23, 2018, with an award potentially being made around August 28, 2018. If a solicitation is issued, the Government will do so in accordance with Federal Acquisition Circular (FAC) 2005-94, 2005-95/ 01-19-2017, FAR Part 13.5, Simplified Acquisition Procedures. The North American Industry Classification System (NAICS) number is 339112 and the NAICS size is 1,000 employees. Information gathered during this request for information may be used in a small business set aside. Any concern that believes they are capable and desires to claim preference for small business status must be registered with the SBA at http://web.sba.gov/pro-net/. Any contractor that believes they are capable and desires to claim preference for veteran owned small business status must be registered with the VIP at https://www.vip.vetbiz.gov as an SDVOSB or VOSB. Responses shall include: (1) Business Name and Address, (2) FSS Contract Number (3) Vendor Point of Contact Name, Phone Number and E-mail Address (4) DUNS and NAICS code (5) Business Size (6) Type of Business: service disabled veteran owned, veteran owned small business, 8a, HUB Zone, woman-owned, etc. (7) If the concern is the manufacture and the sole distributor, the concern shall provide documentation supporting the claim. Contractors that deem themselves capable of meeting the requirement shall provide the below information to Jordan Carlsen, Contract Specialist, at Jordan.Carlsen2@va.gov no-later-than Tuesday, August 21, 2018 at 4:30PM, EST. Contractor must be registered with https://www.sam.gov Supplies are to include only TAA compliant items. Items that are not TAA compliant must be clearly identified by the contractor on the contractor s quote should a solicitation be subsequently issued. FOB is Destination. Performance Period: 10/01/2018 to 9/30/2023 Delivery is to: VAMC Detroit 4646 John R Street Detroit, MI 48201-1916 SEE SOW BELOW FOR DETAILS OF THIS REQUIREMENT Statement of Work Dosimetry Badges Definition: For purposes of this requirement, the term "personnel dosimeter" is an individual monitoring device that requires processing to determine the radiation dose and that is used by John D. Dingell VA Medical Center- Detroit to comply with 10CFR20.1201, with other applicable provisions of Title 10 Chapter 20, or with conditions specified in the license. This Statement of Work is for a requirement for a Base, plus 4 option years, for the purchase of radiation badges for the John D. Dingell VAMC. As required by federal law specified in 10CFR20.1501, personnel dosimeters must be processed and evaluated by a dosimetry processor-- Holding current personnel dosimetry accreditation from the National Voluntary Laboratory Accreditation Program (NVLAP) of the National Institute of Standards and Technology; and Approved in this accreditation process for the type of radiation or radiations included in the NVLAP program that most closely approximates the type of radiation or radiations for which the individual wearing the dosimeter is monitored. REQUESTS FOR SERVICE ROUTINE VA may request routine individual monitoring devices that report dose as follows: Approximately 250 per month personnel dosimeters to be processed and holding current NVLAP accreditation in ANSI N13.11 categories IA, IIA, IIIA, IV, and V. Approximately 275 quarterly personnel dosimeters to be processed and holding current NVLAP accreditation in ANSI N13.11 categories IA, IIA, IIIA, IV, and V. Approximately 10 finger ring dosimeters to be processed monthly. Approximately 5 finger ring dosimeters to be processed quarterly. Some individuals may have multiple badges. Fetal monitoring devices may be required. Personnel dosimeter results will be reported in units of mrem for shallow dose, lens of eye dose and deep dose. Routine dose reports should be furnished within 30 days of processing. Annual dose reports shall be provided on NRC Form 5, or equivalent, for all individuals monitored in the prior year. EMERGENCY VA may request emergency processing of individual monitoring devices. Reports of emergency processing should be furnished within 24 hours of processing. OTHER REQUIREMENTS: The Contractor shall immediately notify the contracting officer representative if any of the following occur: NVLAP accreditation is rescinded in any of the required categories listed above. The Contractor revises their Quality Assurance practices in such a way as to decrease the effectiveness of a NVLAP accredited program. The Contractor modifies the dosimeter in such a way as to invalidate its NVLAP accreditation. Any dosimeter indicates > 5000 mrem. Additionally, the Contractor shall: Ensure that dosimeters are zeroed prior to shipment, Apply for, obtain and maintain any certifications required by governing federal, state, or local agencies, Participate in performance evaluation programs required by federal, state, or local agencies, Comply with the requirements of an approved quality assurance program plan, Provide VA the current revision of the quality assurance program plan, if requested, Provide VA with sufficient QC and calibration data to assure that analytical results of dosimetry are accurate and analyzed within the established envelope of quality, if requested, Provide the value of the Le/ MDA, in the same units as results (mrem), Provide the signature of the person responsible for the quality of the report or his/her designee, and Read the dosimeters on an as needed basis. Cognizant regulatory agencies and VA representatives shall be granted access to Contractor facilities, personnel, equipment, and records related to VA utilized vendor provided services. Contractor shall be responsible for notifying NVLAP, and VA prior to making a change in dosimeters used, or other equipment, materials, software, analysis models, or techniques that were considered to be primary components of a dosimetry program.
