- Attachment 1 - Salient Characteristics Request for Quotes (RFQ) JBSA Lackland Map Short Term Pass Request Form Questions and Answers Template This is a requirement for a "brand name or equal" Haike Ceiling Fans as indicated below. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a separate Request for Proposal (RFP) will not be issued. The solicitation number is #FA3016-18-U-0336 and is issued as a Request for Quotation (RFQ). See attachment for the RFQ. This purchase is a Small Business Set-Aside. This NAICS code is 335210 and the size standard is 1,500 employees. Please respond to this RFQ by emailing response to the Primary and Secondary point of contact (POC) below. When submitting questions please use the sample format provided in the attachments. For site visits a base pass is required to gain access. Please utilize the pass request format and submit 24 hrs. prior or earlier to obtain a pass. Passes can be picked up at the JBSA Lackland Visitor Center, see below for request format. Site Visit.
