Description
This is a sources sought for the PP VAMC to determine the availability of potential sources (and their size classification relative to NAICS 562998 having the skills and capabilities necessary to provide kitchen grease removal for Nutrition & Food Services in accordance with the attached SOW. All interested vendors are invited to provide information to contribute to this market survey/sources sought including, commercial market information. This is not a solicitation announcement. No award will be made from this sources sought. Requested information should be submitted by email to vanessa.jacobs@va.gov by 12:00pm EST August 27, 2018. Provide only the requested information below. REQUESTED INFORMATION STATEMENT OF CABABILITY: Submit a brief [ Five (5) pages or less] capability statement explaining your capability to perform services for the DC VAMC. Include past experience in performing these services for the VA, other Government (Federal or State) agency, or for a private medical facility.. BUSINESS SIZE AND SOCIO-ECONOMIC STATUS: (a) Indicate whether your business is large or small (b) If small, indicate if your firm your firm is a certified Service-Disabled Veteran Owned Small Business (SDVOSB) or Veteran Owned Small Business (VOSB) (c) Include the DUNS number of your firm. (h) Provide your firms FSS or GSA Contract number if applicable. STATEMENT OF WORK PURPOSE: This purchase request a contractor to remove grease from traps in the Nutrition and Food Service Building 17-H.on a monthly basis The vendor will provide this service using vendor's equipment, material, and labor to pump out and remove grease from kitchen trap in Nutrition & Food Services in building 17H. GOVERNMENT REQUIREMENT: Maintaining this equipment fulfills the daily routine and urgent requirements for the Nutrition and Food Service in Building 17-H in Perry Point, MD. The service makes it feasible to provide meals for the veterans in the medical facility. Also it keeps the Medical Center in compliance with our sewer discharge agreement with the Town of Perryville. STANDARDS: Work to be performed on Weekdays only, between the hours of 08:00 a.m. to 4:30 p.m., excluding Federal holidays. Vendor must be factory trained and authorized to perform all maintenance and service tasks. Vendor is to provide all necessary transportation, parts, supplies, materials and labor to accomplish all tasks. Work to be accomplished is to perform preventative maintenance, repair, or restore equipment to within manufacturer s specifications of normal operating parameters and safety policy. Waste is to be removed and disposed of in an authorized matter. Any spills or damages are to be cleaned or repaired by the vendor. REQUIRED REPORTING: a. Contractor personnel are required to check in with the chief of the using service prior to starting work and check out at the end of the work day. b. Chemicals: Contractor will furnish a list to the chief of the using service, prior to performance of any work, the trade names (if any) and chemical names and strengths used of all chemicals, along with antidotes, that are used in performance of services. c. Badging: Vendor is to have identification badging visible on their person while on VA premises. d. Security: All vendors and visitors are subject to Federal Installation Security Requirements, including inspection of any containers, equipment or vehicles as required. e. VA Security IT Checklist 6500.6 statement: The C&A requirements do not apply, and that a Security Accreditation Package is not required. SPECIAL PROVISIONS: None LOCATION: Buildings 17H.
