- Center Physician Scope ParadigmWorks Group, Inc. is contracted by the U.S. Department of Labor for the operation of the Carville Job Corps Center, a residential adolescent training facility located at 5465 Point Clair Road, Carville, LA 70721. ParadigmWorks Group, Inc. is currently soliciting written bids for Center Physician. Carville Job Corps Center is a federal funded facility. All quotes must be submitted in writing on a per hour basis to the Purchasing Department by September 7,2018 by 3:00 p.m. Quotes received after the deadline and/or without all requested documentation will not be considered. Bid must be good for 60 days, after submission. No partial bids will be accepted. Vendor must submit a written bid on company letterhead to include scope of work ID#CJCC8182018-2 at the top each of page. Vendor must provide licenses for doing business in the State of Louisiana, all employee licenses required for performing work in the state of Louisiana, certificate of liability insurance and list of references if no past experience with Carville Job Corps Center.
