- Payment Information Kosher Certification Dairy Delivery Information RFQ DAIRY Q1 FY19 Dairy Combined Solicitation Q1 FY19 This is a Combined Synopsis/Solicitation for commercial items prepared in accordance with the format in Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation of offers that are being requested. The solicitation number is 15BNYM18Q00000023 and this solicitation is issued as a Request for Quote (RFQ). PLEASE READ THROUGH ITS ENTIRITY. Solicitation is for 1st Quarter Subsistence, FY 2019 (Dairy Items). For a list, description and total quantities of all items, refer to Section 2 Commodity or Services Schedule. Solicitation is a 100% small business set aside. Requirement may be awarded as all or none. Please quote all items for consideration. The NAICS for this requirement is 311511. Terms of delivery are F.O.B. destination to: Metropolitan Correctional Center New York 150 Park Row New York, NY 10007 Contact: Rocco Lupo, Food Services Administrator 646-836-6344 Steven Chambers, Warehouse Supervisor 646-836-6951 The deliveries are expected weekly from October 1, 2018, through December 31, 2018 (Mondays and Thursdays between 7:00 a.m. to 9:00 a.m.). Any items shipped before or after the required delivery time-frame will be returned at vendor's expense. Upon Award, the Food Service Warehouse must be contacted prior to delivery for approval or adjustments in delivery schedule using the information above. Quoter(s) are advised the government reserves the right to make single or multiple awards. The government will award to the responsible quoter(s) whose quote, is conforming to the solicitation. This request for quote may be awarded as an all or none award. The aggregate total of all line items may be used to determine the award for this RFQ. Evaluation Factors are past performance, price, and delivery. Past performance is as important as price and non-price related factors. Award will be based on the best value to the government considering the evaluation factors listed above. Best value may be determined using tradeoffs among the evaluation factors, so that award may be made to other than the lowest-priced offer. Vendors who submit an offer concurs that delivery date can be met, unless otherwise noted. Products other than canned goods must be 3 months or younger (canned good must be 1 year or younger) upon delivery with the date (including expiration date, best by date, sell by date, use by date or consumer by date) on the label. No stamped/written dates allowed. PAST PERFORMANCE RECORD OF "FAILURE TO DELIVER" ON PREVIOUS AWARDS MAY RESULT IN A LESS THAN FAVORABLE PERFORMANCE RATING. Government intends to make an All or None Award - failure to quote on all line items may result in not being considered. Price will be evaluated on the aggregate total. Piggy back offers will NOT be evaluated and therefore NOT considered for award. DO NOT CHANGE CASE COUNTS!! YOUR OFFER WILL NOT BE CONSIDERED! SUBSTITUTIONS TO SPECIFICATIONS IN LINE ITEMS ARE PROHIBITED (ex. SIZES, CASE COUNTS). CHANGES TO SPECIFICATIONS WILL NOT BE EVALUTED AND THEREFORE, NOT CONSIDERED FOR AWARD. THE GOVERNMENT RESERVES THE RIGHT UPON INSPECTION TO REJECT ITEMS IF THEY ARE DAMAGED OR NOT TO THE SPECIFICATIONS AS LISTED IN THE SOCLITATION. ALL ITEMS MUST MEET SPECIFICATIONS. ALL QUOTES MUST BE CLEAR! Do not provide a Unit Price past the 4th decimal point for your quote or that line item will not be considered. For quotes consideration for any kosher items offered, follow the Certified Religious Diet Specifications Quote Sheet in this solicitation. All kosher items offered on this quote must be certified by and accepted by the Orthodox Kosher Certification Agency from the United States or Canada. Quoter(s) MUST include a Letter of Certification from the religious certification agency with all quote(s) submitted. Please submit the Kosher Certification(s) (date valid from time of quotes submittal through the delivery quarter) with your quote. Follow the Certified Religious Diet Specifications Quote Sheet. The completed package should be returned no later than 08:00 a.m. Eastern Standard Time, August 30, 2018. Vendors shall submit signed and dated offers to Richard Ovalle, Contracting Officer by email to rovalle@bop.gov ONLY. Fax, hand delivery, nor mail-in will be accepted. Only quotes submitted to rovalle@bop.gov will be considered. Be sure to complete blocks 17a, 23, 24, 30a, 30b, and 30c of SF-1449 and the continuation sheets. Please include your TELEPHONE number, e-mail address AND DUNS in block 17a. ALL offers shall be submitted on the specific RFQ/SF1449. Information will be posted to the General Services Administration Federal Business Opportunities website: http://www.fbo.gov. All future information about this acquisition, including amendments, cancellations, award letter and low quote(s) will also be distributed through this site. Interested parties are responsible for monitoring this site to ensure they have the most up-to-date information about this solicitation. The anticipated award date is September 14, 2018. Faith Based and Community Based Organizations have the right to submit offers or quotes equally with other organizations for contracts for which they are eligible.
