- STATEMENT OF WORK (SOW) 1. Point of Contact (POC). Name: Section: Address: Phone Number: Fax Number: E-Mail Address: 2. Contract Title. Poster Advertising for Veterans Crisis Line 3. Background. The Suicide Prevention Team at the William S. Middleton Memorial Veterans Hospital in Madison, WI was able to put a billboard up in Madison promoting the Veterans Crisis Line during FY 17. We would like to expand this idea and put posters (half the size as billboards) up in each city that houses one of our CBOC s. 4. Scope. The advertising vendor will be expected to print the image designed by National Office, install the poster on one of their boards, and leave the poster up for 4 weeks during September, 2018 (Suicide Prevention Month). 5. Specific Tasks. Advertising provider will be required to print five poster sized images (approx.. 10 6 X 22 9 ). These images are not to be shared with anyone else and are for the purpose of printing the poster image only. Contractor will then install advertising on Contractor s provided sites, no sites will be provided by the Government, with one located in Rockford, IL; one in Freeport, IL; one in Janesville, WI; one in Beaver Dam, WI; and one in Baraboo, WI. See table below for install dates. These posters will remain posted for the entire four weeks from the date of installation. If the location of these posters has not been rented following the completion of this four weeks; these posters shall remain until the space has been rented by another company at no additional cost to the government. Location Install Janesville, WI 1-Oct Freeport, IL 8-Oct Rockford, IL 1-Oct Beaver Dam, WI 24-Sep Baraboo, WI 1-Oct 6. Performance Monitoring. The POC, or his/her delegate, will contact the local VA CBOC Manager to request that they check that the image has been installed and remained installed during the entire contract period. 7. Security Requirements. No contractor staff will be required to provide service on Madison VA property. There are no security requirements. 8. Government-Furnished Equipment (GFE)/Government-Furnished Information (GFI). No Government-Furnished Equipment will be furnished or required. 9. Inspection and Acceptance Criteria. The POC, or his/her delegate, will contact the local VA CBOC Manager to request that they check that the image is acceptable without any signs of interference or distortion of picture. 11. Place of Performance. Work will occur at locations owned by the Advertising provider in Rockford, IL, Freeport, IL, Janesville, WI, Beaver Dam, WI, and Baraboo, WI. Any travel and/or delivery to and from work location is the responsibility of the contractor. 12. Period of Performance. All work is shall begin prior to September 24, 2018 to allow for adequate time to create and post the advertisement. The Government would be in agreement if the Contractor can display the advertisement prior to September 24, 2018 but not before September 1, 2018 at no additional cost; advertisement is to be displayed for four (4) continuous weeks from date installed. If the space where these posters are located is not rented following this date VA would agree that posters can remain until spots are rented at no additional cost to the Government.
