Description
The Department of Veterans Affairs, Network Contracting Office 10, VA Northern Indiana health Care System, 1700 E 38th Street, Marion, IN 46953 intends to solicit offers for the procurement, delivery set up and installation of furniture and furnishings. Prosthetics & Sensory Aid Services in conjunction with the Interior Design team are requesting furnishings to support the activation of a brand-new Prosthetics & Sensory Aid Orthotics Central Fabrication Unit. The project is approximately a 9,260-sq. foot space in Building 79 at the VANIHCS, Marion Campus consisting of multiple administrative office spaces, various fabrication and storage spaces, a receiving area, staff break room and locker rooms. This statement of work describes the requirements for the procurement, delivery, set up and installation of furniture and furnishings, for the VA Northern Indiana Health Care System (VANIHCS), Marion Campus, Building 79 Prosthetics & Sensory Aid Orthotics Central Fabrication Unit Project #610-17-510. 2.0 BACKGROUND. Prosthetics & Sensory Aid Services in conjunction with the Interior Design team are requesting furnishings to support the activation of a brand-new Prosthetics & Sensory Aid Orthotics Central Fabrication Unit. The project is approximately a 9,260-sq. foot space in Building 79 at the VANIHCS, Marion Campus consisting of multiple administrative office spaces, various fabrication and storage spaces, a receiving area, staff break room and locker rooms. 3.0 SCOPE OF WORK. This project shall consist primarily of delivery, assembly and installation of all furniture and furnishings outlined in this document. The Awarded Vendor shall include the removal of all related shipping debris and construction cleanup associated with the installation of the furniture and furnishings for the VANIHCS Building 79 Prosthetics & Sensory Aid Orthotics Central Fabrication Unit Project #610-17-510. 3.1 The Contractor shall provide the following products and related services. 3.1.1 Manufacturing and delivery of products. 3.1.1.1 The Contractor shall be responsible for any damage to the furniture that occurs due to Vendor/freight error or neglect. The Contractor will be responsible for the coordination for delivery, receiving and installation to the VANIHCS. 3.1.1.2 The Contractor shall submit an estimated schedule of delivery and installation. 3.1.2 Standard installation. 3.1.2.1 All work is to be completed during normal business hours. 3.1.2.2 Installation includes the removal and disposal of all trash/packaging associate with the install. 3.2 Preliminary furniture requirements are defined in the documents listed below. 3.3 This is a brand-name or equal procurement. All products shall be brand name or equal to the products listed in this document. Products shall comply with the salient characteristics listed in the Furniture Illustration Sheets and the requirements as defined in Section 4. If a product listed in the Furniture Illustration Sheets has been discontinued or is no longer available, the Contractor shall provide a currently available product that is equal in terms of essential salient characteristics to the product in the Furniture Illustration Sheets. 3.4 In accordance with FAR 52.211-6, the Government has elected to use brand names and specific products in an effort to assist the Contractor in the bidding of this project. The brand names and products specified are being used to establish a minimum quality level for various items and products. The Government does not intend or imply that by providing vendors or manufacturers names in the specification that subject products or items are the only items that will be acceptable to the Government. All products and items, whether named or not named, must clearly meet the salient characteristics of the specifications identified. 4.0 PRODUCT. All furniture must meet the following salient characteristics as specified below and in Technical Specifications section of this document. 4.1.1 Colors and Fabrics. All exterior wood surfaces shall have a manufacturer s standard finish that is the same as, or a close match to, the finish specified on the Furniture Illustration Sheets. Fabric colors and patterns defined in the on the Furniture Illustration Sheets are preliminary recommendations and are not mandatory. However, the fabrics cited on the Furniture Illustration Sheets should be provided unless they are unavailable, either because the fabric is no longer being manufactured, or because the lead time for manufacturing the fabric would delay product delivery in accordance with the schedule. In that case, alternate fabrics shall be of the same or higher grade than the fabrics cited on the Furniture Illustration Sheets. Alternate fabrics shall be the same or similar color, material, pattern, scale, and texture as the fabrics specified on the Furniture Illustration Sheets. All fabric and finish color selections shall be selected and approved by the VANIHCS Interior Designer. 4.1.2 All products shall be designed with durability to meet the demands of the healthcare environment, including abrasion resistance and anti-microbial finish to support the reduction of fungal growth, spores from germs, microbes and bacteria, and provide tight cushion or clean out space to assist with the cleaning of products, and prevention of buildup of debris and germs. 4.1.3 All finishes shall be engineered for heavy use and frequent cleaning using hospital-grade cleaning substances. 4.1.4 Seating Fabrics: All seating fabrics shall comply with the following minimum standards. If the fabrics listed on the Furniture Illustration Sheet is no longer. 4.1.4.1 100% PVC free 4.1.4.2 Bleach Cleanable 4.1.4.3 Anti-Microbial 4.1.4.4 ASTM D4167 Wyzenbeck Method, minimum 100,000 double rubs 4.1.4.5 ASTM D3511 Class 4, Brush Pill Test 4.1.4.6 Colorfastness to Light, AATCC 16 option 1 or 3, Grade 4 minimum at 40 hours 4.1.4.7 California Technical Bulletin 117 4.1.5 All products within each room or space shall have compatible finishes and colors to provide an aesthetically pleasing solution. 4.1.6 Dimensions. All dimensions defined in the SOW, the Furniture Illustration Sheets, and on the drawings, are nominal dimensions. The actual dimensions of each product or component may vary slightly from the nominal dimensions listed, as long as the products provide the same functionality as products of the exact dimensions, and as long as the products fit within each room or space. 4.1.7 Warranty. All products shall be designed for healthcare application and covered with manufacturer s standard commercial warranty for healthcare use 3 shift, 24-7/365 days, the product is free from defects in materials and workmanship (including shipping, parts, and labor for the repair or replacement of defective item). Specific warranty requirements shall be listed below. 4.1.7.1 Office Desk & Storage Units: Laminates and wood veneers minimum 12-year warranty; Paint colorfastness minimum 5-year warranty; Pneumatic door assist mechanism minimum 5-year warranty; Power supply on light minimum 5-year warranty; Light fixture minimum 12-year warranty 4.1.7.2Monitor Arms: minimum 10 years 4.1.7.3 Office Seating: Textile & Cushions 5 years of single shift use; All other components 15 years 24/7 use 4.1.7.4 Seating, metal frame: Seating foam minimum 5-year warranty; Fabric and Finish minimum 3-year warranty 4.1.7.5 Bulletin Boards: minimum 3-year warranty 4.1.8 Sustainability. All products shall be indoor air quality and low emitting VOC requirements in accordance with the Greenguard Indoor Air Quality Certification program, or an equivalent or better environmental standard. The Contractor shall provide documentation showing that their products are Greenguard certified or are certified in accordance with the equivalent or better standard. 4.2 Salient Characteristics. All bids shall address and meet all salient characteristics defined in Technical Specification section and on the Furniture Illustration Sheets. *** FAR 52.211-6 Brand Name or Equal*** FOR ALL PRODUCTS: Must meet salient characteristics outlined above when quoting an or equal item FAR 52.211-6 Brand Name or Equal. Or equal items quoted should be similar to the requested dimensions, color scheme, function, and quality. Or equal products must include manufacturer s product brochure including specification information and all proposed finish selections. Actual finish samples if quoting an or equal item must be submitted for review by VANIHCS Interior Designer for technical and aesthetic review. FAILURE TO CLEARLY NOTE THAT THE PRODUCTS BEING QUOTED ARE THE REQUESTED BRAND NAME, OR FAILURE TO SUPPLY OR EQUAL PRODUCTS SPECIFICATIONS AND INFORMATIONAL LITERATURE SHALL CONSTITUTE A NON-RESPONSIVE BID AND WILL NOT BE FURTHER EVALUATED FOR AWARD. 5.0 DELIVERY/INSTALLATION. Delivery, set-up, and installation shall be coordinated with the VANIHCS Interior Design, Erik Davidson, Erik.Davidson@va.gov, 765-674-3321 ext. 73713. 5.1 Project installation 5.1.1 The Contractor shall submit an installation schedule for approval. 5.1.2 The Contractor shall coordinate with facility on staging needs and parking prior to installation dates. 5.1.3 Protect adjacent existing and newly placed construction, equipment and finishes as much as possible during installation to limit damage. The Contractor shall be responsible for any damage to the building that occurs due to contractor error or neglect. 5.1.4 The Contractor is responsible for the clean-up of each work area and for proper disposal of all packing/crating materials and will have limited access to cardboard recycling, metal recycling and garbage dumpsters. 5.1.5 The Contractor shall furnish all supplies, equipment, facilities and services required to perform the service under this contract. 5.1.6 Any painting and patching, if required, will be performed by the VANIHCS Painters. 5.2 Delivery. 5.2.1 Deliver materials to job in manufacturer's original sealed containers with brand name marked thereon. Protect materials from damage. 5.2.2 Package to prevent damage or deterioration during shipment, handling, storage and installation. Maintain protective covering in place and in good repair until removal is necessary. 5.2.3 Deliver only when the site is ready for installation work to proceed. 5.2.4 Preferred delivery of product to be coordinated with VANIHCS Interior Designer. 5.3 Period of Performance. Contract shall be completed by 1 October 2018. 5.4 Place of Performance. VA Northern Indiana Health Care System 1700 E 38th Street/B-79 Marion, IN 46953 5.5 Operational Hours. All work is to be completed during normal business hours. Facility will be available for deliveries and installation between the hours of 0800 and 1600 M-F, excluding Federal Holidays. 5.6 GOVERNMENT-FURNISHED EQUIPMENT, BADGE, KEYS AND/OR FACILITIES PROVIDED. N/A 5.7 CONTRACTOR FURNISHED ITEMS AND RESPONSIBILITIES. The Contractor shall furnish all supplies, equipment, facilities and services required to perform the service under this contract. The Contractor shall perform all work in accordance with this Performance Work Statement to include the additional new/ refurbished furniture. 5.8 SECURITY REQUIREMENTS. 5.8.1 Installation passes. The contractor will coordinate for the issuance of required installation passes to contractor personnel. Contract personnel shall wear a badge that clearly identifies them as a contract employee. The badge will contain a picture, name of employee, and Contractor's name. Badges shall be worn on the outer garment in full view at all times, attached to the outer shirt or jacket pocket by a button or clip or worn around the neck secured by an appropriate identification card lanyard. 5.8.2 The Contractor shall not disclose and must safeguard procurement sensitive information, computer systems and data, Privacy Act data, and Government personnel work products that are obtained or generated in the performance of this contract. This includes dissemination of protocols and papers not generally available through the public literature. 5.8.3 Company will return all issued VA photo identification badges and passes when the contract is completed. 5.9 NON-PERSONAL SERVICE STATEMENT. Contractor employees performing services under this order will be controlled, directed and supervised at all times by management personnel of the contractor. Contractor employees will perform independent of and without the supervision of any Government official. Actions of contractor employees may not be interpreted or implemented in any manner that results in any contractor employee creating or modifying Federal policy, obligating the appropriated funds of the U.S. Government, overseeing the work of Federal employees, providing direct personal services to any Federal employee or otherwise violating the prohibitions set forth in Parts 7.5 and 37.1 of the Federal Acquisition Regulation (FAR). The Government will control access to the facility and perform the inspection and acceptance of the completed work. 5.10 Contract Officer Representative (COR). The COR monitors all technical aspects of the contract and assists in contract administration. The COR is authorized to perform the following functions: assure that the Contractor performs the technical requirements of the contract: perform inspections necessary in connection with contract performance: maintain written and oral communications with the Contractor concerning technical aspects of the contract: issue written interpretations of technical requirements, including Government drawings, designs, specifications: monitor Contractor's performance and notifies both the Contracting Officer and Contractor of any deficiencies; coordinate availability of government furnished property, and provide site entry of Contractor personnel. The COR is not authorized to change any of the terms and conditions of the resulting order. 6.0 SUBMISSION OF QUESTIONS. All questions submitted for this solicitation must be electronically sent to the Contracting Officer via email. 7.0 SUBMISSION OF QUOTATION. Quote Submission: The contractors shall submit complete emailed electronic copy portable document format (.pdf) or Microsoft Excel (.xls) format of: Volume 1: (technical) a. Drawings, (floorplans, isometrics, shop drawings, typicals etc.) of proposed furniture items with dimensions per bill of materials with product subtotal b. Complete manufacturer s product specifications c. Quantity of man hours for labor/installation d. Product literature e. Finish samples f. Sustainability certification documents g. Complete testing results / certificates h. Warranty i. Self-certifying statement confirming ability to meet project deadlines. The medical center expects a delivery by 1 October 2018. Volume 2: (pricing) a. Price quote with separate line for labor/installation 8.0 EVALUATION CRITERIA. Order will be evaluated on: a. The separate, typed document from the VA Solicitation that matches the Solicitation document exactly per each line item. Products or services that include multiple parts shall be listed within each line item. No additional line items will be added. b. PDFs of detailed and dimensioned: Plan, 2-D and 3-D drawings of each product with multiple parts list to represent exactly what is being included in task order and to determine accuracy. c. Cut sheets with product detail for each line item. d. Labor/Services appropriate to the scope of work. (no pricing included) e. Project staffing plan appropriate to scope of work f. Sustainability Certification g. Price h. Business size Preference will be given to SDVOSBs and VOSBs. Per PPM 2016-05 Implementation of the Veterans First Contracting Program as a Result of the U.S. Supreme Court Decision: 38 U.S.C. § 8127 is mandatory, not discretionary and its text requires the Department to apply the Rule of Two to all contracting determinations. Section 1 Office Furniture Technical Specifications Manufacturers: Subject to compliance with requirements, acceptable manufacturers offering products that may be incorporated into the work include, but are not limited to, the following: Steelcase Herman Miller Workrite Ergonomics The following items are specified as Steelcase Answer and Universal Storage Series products. E0090 E0090A E0123 F0420 The Contractor shall provide brand name or equal modular desking components that are available on the Contractor s furniture schedule. These products shall comply with the salient characteristics listed on the Furniture Illustration Sheets and the following requirements. 1. Freestanding Desk Steelcase, Answer Freestanding Series 1.1. Items shall be tested in accordance and comply with ANSI/BIFMA X5.5 Desk/Table Products safety and performance standards and guidelines. 1.2. Work surface materials shall comply with: 1.2.1. ANSI 208.1-2009 Particleboard Classification 1.2.2. ANSI A208.2-2009 Medium Density Fiberboard 1.2.3. ANSI/NEMA LD3-2005 High Pressure Decorative Laminates (HPDL) 1.3. Laminate work surfaces (Desk & Return) shall be a minimum of 1 1/8-inch-thick top constructed of a Grade M2 or M3 particleboard or a 45-lb. medium density fiberboard core with a high-pressure laminate (HPL) on the face, a backer material on the underside, and 3mm square edge detail 1.4. Structural supports must be welded assemblies of steel components. Leveling glides on end panels should provide at least a 1-1/2 adjustment range. 1.6. Provide half-height modesty panels on all freestanding desks constructed of a minimum 22-gauge steel. 2. Storage Steelcase, Universal Storage Series 2.1. Items shall be tested in accordance and comply with ANSI/BIFMA X5.9 Storage Unit safety and performance standards and guidelines. 2.2. Fixed pedestals should be 22-gauge steel unibody construction with 3 H base integral to cabinet and depth of pedestal shall match the depth of work surface it supports. Pedestals shall feature 1-1/2 tall adjustable leveling glides and mounting features to enable them to be attached to support work surfaces. Drawer fronts should feature flush front with full-width integral pull. Transparent plastic label holders that fit inside integral pull should be provided. Drawer suspensions shall be full extension steel ball bearings, for full depth access to drawer contents. Provide one divider and one pencil tray in each 6 H box drawer. Provide one hanging folder bar for accommodating side-to-side filing of letter-, A4-, and legal-size hanging folders in each 12 H file drawer. 2.3. Lateral files should be 22-gauge steel unibody construction with 3 H base integral to cabinet. Lateral files shall feature 1- 1/2 tall adjustable leveling glides and have mounting features to enable them to be attached to support work surfaces. Overhead Storage should be constructed of 18 to 20-gauge steel components with an over the case door construction with a ball bearing slide mechanism to provide smooth door operation. Bins which are 60 W or more should feature two independent doors with a center vertical support between. Provide minimum of (2) two dividers with bin. Assist mechanism for bins should consist of pneumatic cylinder which slowly closes the door, preventing accidental slamming, and a spring which assists the user in opening the door to a fully open, recessed position. The force required to open and close bin doors should not exceed 5 lbs. and should be operable with one hand in accordance with ADA standards. Hutch kit should be provided for mounting bin above desktops. Vertical clearance between the work surface and the hutch-mounted bin should be a minimum 21 H.
