Page 1 of 6 This is a SOURCES SOUGHT ANNOUNCEMENT ONLY. It is neither a solicitation announcement nor a request for proposals or quotes and does not obligate the Government to award a contract. Requests for a solicitation will not receive a response. Responses to this Sources Sought must be in writing. The purpose of this Sources Sought Announcement is for market research to make appropriate acquisition decisions and to gain knowledge of potential qualified Service Disabled Veteran Owned Small Businesses and Veteran Owned Small Businesses. Responses to this notice shall include the following: (a) company name b) address (c) point of contact (d) phone, fax, and email (e) DUNS number (f) Cage Code (g) Tax ID Number (h) Type of small business, e.g. Services Disabled Veteran Owned Small Business, Veteran-owned small business, and must provide a capability statement that addresses the organizations qualifications and ability to perform as a contractor for the work described below. In addition, Service Disabled Veteran Owned Businesses or Veteran Owned Businesses who respond to a solicitation on this project must have an active/verified registration in VetBiz, located at http://vip.vetbiz.gov. Description: The Government is interested in gaining knowledge of potential businesses that would be able to distribute or manufacture the Brand name or equal. Responses due: 9/5/2018 at 1200 EST BRAND NAME OR EQUAL ERIE Veterans Affairs Medical Center 135 East 38th Street Erie, PA 16504 STATEMENT OF WORK For Boiler Plant Crane Procure and Install SERVICES PROVIDE: The contractor shall provide all necessary labor, equipment and materials necessary to a 2 ton monorail crane system at the Erie VA Medical Center 562 (EVAMC) Main Campus, 135 E 38th St, Erie Pennsylvania, 16504. PROJECT SCOPE GENERAL: Erie Veterans Affairs Medical Center is located in Erie, PA. EVAMC treats eligible veterans for various conditions. EVAMC provides inpatient, outpatient and residential services. This contract is to procure and install a 2 ton lifting crane, a monorail system and complete electrical power connections. SPECIFICS: Load capacity of the electric chain hoist shall have a minimum rating of 2 tons and capable of passing a 125% load test. Other specifications for the provided electric chain hoist shall meet the following requirements: 21 feet of lift Single speed hoist of approximately 7 fpm Chain container Pendant control Top hook Load capacity of the monorail track shall have a minimum rating of 2 tons and equal to or greater than the capacity of the electric chain hoist in A above and capable of passing a 125% load test. Other specifications for the provided monorail system shall meet the following requirements: Monorail is to be supported from a 5 flange girders pre-engineered and integrated into the building construction Supported at 21 centers using pre-engineered bolt hangers 20 grade 8 threaded rods adjustable height to hang below the pipes End stops Festoon cable for hoist power Sway bracing All hardware is structural grade 8 or A325. Contractor shall provide a power source rated to match the lift s needs with a lockable disconnect accessible from the ground. Contractor shall provide conduit with J Box to be located on the wall at the end of the monorail. Contractor shall be responsible to furnish and install of all needed rigid metal conduit, mounting hardware, breaker and wire from electrical panel to the crane junction box. All items shall be sized per the National Electrical Code. The landing point for the wiring is in panel L6. Contractor shall install a permanent personnel fall protection tie off point on the main level. The contractor shall provide all services in manner which will impact the EVAMC patients and employees the least. All work is to be done during normal EVAMC working hours. Normal EVAMC working hours are Monday-Friday 7 AM-4:00 PM, excluding Federal holidays. Contractor shall provide a certified load test to 125% of rated load for 30 minutes following installation. The contractor shall provide the following documents within 2 weeks following contract award: Schedule Construction safety plan Contractor s employee roster SDS for products to be used Specifications for products to be Completed VA Privacy Training documentation for employees that will work onsite The contractor shall not mobilize on site less than 1 week following schedule approval of the schedule listed in III.J.1. The contractor shall not construct barricades or remove an area from service, until specifically directed by the COR. The contractor shall not restrict access to the medical center, lots drives or grounds. Parking shall be in normal parking areas unless COR has given permission to use an alternate location. Dust and debris shall be controlled during cutting and grinding evolutions. All trash and debris shall be removed from the work site daily and disposed of. The contractor is responsible for transportation of personnel, tools, materials, gauges, etc. necessary for the performance of this contract. The contractor is responsible for safe and legal transport and disposal of all items necessary for the performance of this contract. All specification sheets, SDS, chemical precautions, etc. shall be submitted to CO and COR for evaluation with bid submittal. Substitute chemical compositions may be submitted to CO and COR for evaluation of suitability for this contract, prior to use. Type of Contract: Firm-Fixed-Price. Delivery: Delivery shall be F.O.B. The Government is not obligated to nor will it pay for or reimburse any costs associated with responding to this sources sought synopsis request. This notice shall not be construed as a commitment by the Government to issue a solicitation or ultimately award a contract, nor does it restrict the Government to a particular acquisition approach. The Government will in no way be bound to this information if any solicitation is issued. The North American Classification System (NAICS) code for this acquisition is 333923 (size standard to be considered small business is 1,250 employees). Notice to Potential Offerors: All Offerors who provide goods or services to the United States Federal Government must be registered in the System Award Management (SAM located on the web at www.sam.gov). It is desirable that any Offeror to have completed their business Online Representations and Certifications Application in the System for Award Management (SAM). Additionally, all companies that are distributors must have a manufacturers letter stating that they are an authorized distributor or reseller to prevent any Gray Market warranty issues.
