Combined Synopsis/Solicitation AMENDED ITEMS BELOW: This is a combined synopsis/solicitation for Brand Name only INTERCOM commercial items under NAICS 334290 or similar coding, prepared in accordance with FAR Subpart 12.6, as supplemented with additional information included in this notice for the OPERATING-SURGICAL ROOM, VHA Indianapolis, Indiana. Please note that this solicitation is a small business set-aside. Responses should include the following information: (1) Business Name and Address, (2) Point of Contact Name and Phone Number and E-mail Address, (3) GSA/FSS Contract Number (if any), (4) DUNS and NAICS codes, (5) Small Business type. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-83. All responsible sources may submit a proposal which shall be considered by the contracting officer. To be technically acceptable, items must be TAA Compliant. FOB Destination. Delivery and installation are needed within 60 days. Items will be shipped to and installed at: Richard L. Roudebush VA Medical Center OPERATING-SURGICAL ROOM 1481 West 10th Street Indianapolis, Indiana 46202-2803 All questions shall be sent in writing to Terry Van Allen, Contracting Specialist, at terry.vanallen@va.gov, who will provide written answers from the technical reviewers. QUOTES shall be submitted to Terry Van Allen, Contracting Specialist, at terry.vanallen@va.gov no-later- than Wednesday, September 5, 2018 at 9:00 am Eastern Time. There following amended statement is made as a result of the SITE VISIT on August 22, 2018. Please see the Brand Descriptions below along with Attachments for Product Specifications: AMENDED STATEMENT ON ITEMS: The 3 desktops that were identified on the walkthrough will be the IP Desktop Master Station w/ handset #1008001000. The 10 that changed in the OR Rooms will be IP Desktop Master Station # 1008000000, which has no handset. Since these are not going to securely mounted to the wall anymore, we are adding 1 TKIS-2 VoIP intercom module to be in located in the Data Closet, so it is in a secured location. TKIS-2 #1008131020. All of these items will be used in PULSE mode, so no licenses will be needed. The offeror shall provide its DUNS, or if applicable it s DUNS+4 number, with its offer which will be used by the contracting officer to verify that the offeror is registered in the SAM database. Lack of registration in the SAM database will make an offeror ineligible for award. Offerors and contractors may obtain information on registration via the internet at http://fedgov.dnb.com/webform, or if the offeror does not have internet access it may call Dun and Bradstreet at 1-866-705-5711. 52.212-1 - Instructions to Offerors - Commercial Items applies to this acquisition and offerors must comply with all instructions contained within it. 52.212-2 - Evaluation - Commercial Items, the Government will award a purchase order resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation is the lowest price technically acceptable; that is most advantageous to the government; price and other factors considered. 52.212-3 - Offeror Representations and Certifications - Commercial Items. A completed copy of this provision shall be submitted with the offer unless ORCA registered which must be noted in quotation. 52.212-4 - Contract Terms and Conditions - Commercial Items. 52.212-5 - Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items to include: 4, 7, 8, 14, 22, 26, 27, 28, 30, 33, 40, 44, and 50. 52.252-2 - Clauses Incorporated by Reference.
