- This is a REQUEST FOR INFORMATION (RFI) only. Market research is being conducted to determine the amount of qualified and capable businesses that can provide the items requested below. If you believe your firm is able to provide the items requested below, you are encouraged to respond by the due date with all of the required information indicated below in order to be considered. Due date for responses is 1:00PM local time/CST on Monday, September 10, 2018. Email responses to Ryan Buatala at Ryan.Buatala@va.gov. All questions must be submitted in writing to the Contracting Officer in writing via email. Telephone inquiries will NOT be accepted/returned. When submitting a response to this RFI, offerors must include the following: Indicate socio-economic status Include picture of offered items and support literature, brochure etc Indicate any existing Government contract where item(s) may be purchased (i.e. FSS/GSA contract etc) Must be able to provide installation DISCLAIMER-This RFI is issued solely for information and planning purposes only and does not constitute a solicitation. NAICS: 339112 Size Standard: 1,000 employees BRAND NAME OR EQUAL-SALIENT CHARACTERISTICS: Lumenis Moses Pulse 120H Holmium Laser Laser must have capability of power up to 120 Watts. Laser must have operational range of 5 to 80 Hz and 0.2-6.0 joules. Laser must have a fiberoptic port. Laser must have ability to treat urological problems. Laser must have a foot pedal with a minimum of two (2) settings Laser must have the ability to store procedure settings for various providers. Laser must have the ability to integrate suction for percutaneous lithotripsy. Laser must have Moses technology for reduction in retropulsion/migration for more time in contact with stone. Laser must come with safety glasses and safety signage. Onsite installation, calibration, and training service request corresponding to the delivery.
