Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 -- Streamlined Procedures for Evaluation and Solicitation for Commercial Items, in conjunction with FAR -- Part 13 Simplified Acquisition Procedures; and supplemented with additional information included in this notice. This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular 2005-96, dated November 6, 2017. Solicitation 36C26218Q9251 is issued as a Request for Quote (RFQ) and constitutes the only solicitation. The Government intends to award a single Firm-Fixed Price (FFP) award contract. This procurement will be issued under as a Total Service Disabled Veteran Owned Small Business Set-Aside. NAICS Code is 334118 and the Small Business Administration s (SBA) size standard 1000 Employees. The Government is seeking a contract to acquire High Frequency 2 Channel Clinical Audiometers. that at a minimum meets the following salient characteristics with a brand name or equal product for the VA Greater Los Angeles Healthcare System (VAGLAHS): MUST OPERATE IN OTOSUITE SOFTWARE MUST INTEGRATE WITH OTOSUITE PRODUCTS AND NOAH SOFTWARE MUST HAVE ABILITY TO IMPORT DATA TO VA COMPUTERIZED PATIENT RECORD SYSTEM (CPRS) MUST PROVIDE FRESH NOISE STIMULUS: AVAILABLE IN ENTIRE FREQUENCY RANGE WITHIN THE TRANSDUCER SPECIFIED RANGE. (FOR SF 125-12500 HZ, ACCURACY 0.3%) MUST INTEGRATE TINNITUS QUESTIONNAIRES (THI, THS, TFI,) WITH ABILITY TO STORE, REVIEW AND SHARE DATA DURING ASSESSMENT MUST INTEGRATE PEDIATRIC MODULE WITH VISUAL REINFORCEMENT AUDIOMETRY (VRA) MUST BE PC BASED TWO CHANNEL AUDIOMETER-TWO SEPARATE AND IDENTICAL CHANNELS MUST BE AIR, BONE AND SPEECH AUDIOMETER MUST INTEGRATE QUICKSIN TEST MUST INTEGRATE PSYCHOACOUSTICAL TESTS FOR PITCH AND LOUDNESS MATCHING, MINIMUM MASKING LEVEL (MML), AND RESIDUAL INHIBITION (RI) MUST PROVIDE SPECIAL TESTS SUCH AS TEN TEST AND SAL TEST CONTROL PANEL MUST BE CLASSICAL AND TOUCHSCREEN See attached SF 1449 Solicitation for further details.
