Description
Introduction/Description: The Federal Aviation Administration (FAA) is issuing this Request for Offer (RFO) for Occupational Safety and Health Administration (OSHA) Compliant Fall Personal Protection Equipment (PPE). This fall protection equipment is necessary for all field personnel in the FAA who have to work at heights. This requirement is for supplies only. The Government requires equipment that is manufactured by the following five (5) companies: Bullard, 3M DBI-SALA, Ergodyne, Pensafe and Radnor. Standardized equipment is required so that the FAA can provide standardized training, standardized equipment inspections, and tracking of recalls. Standardization enhances the effectiveness of training and equipment inspections and the safety of our workers. Contract Type and Period of Performance: Upon award, the FAA plans to issue one unilateral, fixed price Purchase Order for this equipment with a period of performance of up to six months. This equipment is needed immediately. Nature of Competition: This is a total small business set-aside. The principle North American Industry Classification System (NAICS) code for this effort is 423840 - Industrial Supplies Merchant Wholesalers. The small business size standard for this code is 100 employees. Instructions to Offerors: Attachment A provides a list of the required equipment and their quantities. Attachment B provides the locations that the equipment is to be delivered to. Offerors are to fill out Attachment A with their unit and extended prices for each type of equipment. Offerors are to fill out Attachment B with the estimated FAA arrival dates for each type of equipment. Offerors are to provide all equipment item term and conditions and warranty documentation. Request for Offer Submission Instructions: Proposals are to be submitted via email to Robert.C.Taylor-II@faa.gov to arrive no later than 9:00 AM EST, Monday, September 10, 2018. The proposal submissions are to include the following: 1 - Completed Attachments A and B. 2 - Any equipment terms and conditions and warranties 3 - Cover letter describing the company's qualifications to provide the required equipment and explaining how the Offeror will provide the fall protection equipment no more than three pages in length and in either Word or Adobe Acrobat format. 4 - Copy of SBA 8(a) Certification Form and/or proof of service-disabled veteran-owned small business status (if applicable) Basis of Award: Contract will be awarded to the responsible Offeror whose proposal offers the best value and is most advantageous to the Government. In determining best value, the Government may, in accordance with FAA Acquisition Management System (AMS) 3.2.2 Source Selection, employ a tradeoff process allowing for award to other than the lowest price offeror or other than the highest technically rated offer. Offerors ™ proposals will be evaluated on price and estimated FAA arrival dates. Price is more important than estimated arrival date. However, to the degree that two or more Offerors ™ prices are comparable, arrival date becomes more significant. Past performance will not be evaluated as a separate technical evaluation factor, but will be considered by Contracting Officer as a part of the responsibility determination. An Offeror with a record of unsatisfactory past performance may be determined ineligible for award. Evaluation Factors: Total Equipment Price inclusive of all items Equipment Estimated FAA Arrival Dates Criteria for Evaluating Estimated Arrival Dates: Excellent: Vendor is able to deliver the majority or 75% of the total equipment quantity to the two FAA locations within three weeks of Purchase Order Award and the remaining equipment within five weeks. Good: Vendor is able to deliver nearly all of the total equipment quantity to the two FAA locations within five weeks. Satisfactory: Vendor is able to deliver nearly all of the total equipment quantity to the two FAA locations within within seven weeks. Unsatisfactory: Vendor is not able to meet the criteria for Satisfactory. Note that the FAA reserves the right to terminate an awarded Purchase Order if the awarded company does not make a best efforts attempt to meet the vendor's proposed equipment FAA arrival times. Offerors submitting Proprietary Information must label these materials as Source Selection Sensitive. Questions: Questions concerning this Request for Offer must be submitted by email to Robert C.
