Description
Wage Determination 7/03/2018 Applicable Clauses Performance Work Statement This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation being requested. A written solicitation will not be issued. The solicitation number is W912JB-18-Q-0151. This solicitation is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-89. This requirement is 100% set-aside for small business. The associated NAICS code is 621399 Offices of All Other Miscellaneous Health Practitioners. The small business size standard is $7.5 million. The Michigan Army National Guard (MIARNG) requires support for EOD and Firefighter Occupational Medical Surveillance Examination Program in accordance with the Occupation Safety and Health Association (OSHA) and other regulations. Due to the majority of personnel being spread out across the entire state of Michigan with some areas having distances of greater than 150 miles to an established facility, services should be provided in a mobile setting. This requirement is a firm fixed price. See Performance Work Statement for additional requirements (Performance Work Statement for Medical Surveillance Services, 4 Sept 2018, 14 pages). CLIN Item Qty U/M 0001Medical Surveillance 1 Job 0002Contractor Manpower Reporting 1 Job Offers will be evaluated based on the lowest price technically acceptable, and no negative performance on FAPIIS for the previous three years. Inspection and acceptance will be at Lansing, Michigan. The period of performance is 12 months. APPLICABLE PROVISIONS AND CLAUSES Attachment 2 (Applicable Clauses and Provisions, 4 Sept 2018, 9 pages) lists the clauses to be included in the award (details can be located on http://farsite.hill.af.mil). QUOTATION PREPARATION INSTRUCTIONS: (1)Contact/Company Information point-of-contact name, address, e-mail, and telephone number CAGE code and DUNS number The contracting officer is not responsible for locating or obtaining any information not identified in the quote. (2)Pricing Price quotes must be firm-fixed price. Price quotes must include unit prices and extended amounts for each line item (i.e., multiply the unit price and quantity for each line item). Include the total price (i.e., sum of all extended amounts) in the price quote. Include delivery terms, any discounts, and payment terms (e.g., Net 30) with the price quote. Prices must be valid for 60 days. In accordance with FAR 52.222-42 the Service Contract Labor Standards statute identifies the classes of service employees expected to be employed under the contract. Attachment 3 provides the current Wage Determination (2015-4873, 3 July 2018, 11 pages) to be used in determining the fair labor wages. The most current Wage Determination should be used when preparing the quote. The Contracting Office will use the most current determination at the time of award. (3)Technical information/descriptive literature. Quotes must include technical information (descriptive literature/specifications) that adequately demonstrates their quote meets or is comparable to the salient characteristics requirement as specified in the Performance Work Statement of this solicitation. A general statement of compliance or restatement of the salient characteristics is insufficient. The Contracting Officer is not responsible for locating or obtaining any information not identified in the quote and literature. The Government may not consider quotes that fail to include all of the information requested in this solicitation. Offers are due at 12:00 am, Eastern Time on 14 September 2018. Email quotes directly to bonnie.l.reineer.civ@mail.mil. Questions are due by 4:30 pm, Eastern Time, 10 September 2018, and must be submitted via email. Questions will not be answered via telephone.
