Department of Veterans Affairs, Network Contracting Office 15, Consolidated Mail Out-Patient Pharmacy (CMOP) has an EMERGENCY requirement to procure GLUCOSE SENSORS listed below for delivery to the CHARLESTON CMOP facility: The Product Code for this solicitation is 6515, Medical & Surgical Instruments, Equipment, and Supplies; the NAICS code for this solicitation is NAICSC 325413 (In-Vitro Diagnostic Substance Manufacturing). All responsible sources may submit a quotation, which if timely received, shall be considered by this agency. Submit the RFQ to Lori.NicholsFellows@va.gov The phone number is (913) 684-0154. Solicitation package 36C77018Q782 shall be posted on FedBizOpps on or around September 10, 2018, with an anticipated due date of September 12, 2018. No solicitation packages will be mailed.
