- B.3 Statement of Work The Contractor shall provide all necessary labor, materials and transportation as necessary to deliver dry ice to the government for the period of 10/1/2018 to 3/31/2019 with four (4) option years. Description of items (s): Deliver one (1) 50-pound block of dry ice every Monday. Block shall be sliced into 11/2 layers. Deliver one (1) 50-pound block of dry ice every Wednesday. Block shall be sliced into 11/2 layers. Deliver one (1) 50-pound block of dry ice every Friday. Block shall be sliced into 11/2 layers. Transportation: transportation via refrigerated truck shall be included. Delivery: Location: deliveries shall be made to building 31 of 2215 Fuller Road, Ann Arbor, MI, 48105. Contractors will be met at the building entry and escorted by research staff to the appropriate room. Receiving hours: all deliveries shall be made during normal working hours 8:00 a.m. until 3:30 p.m. Monday, Wednesday and Friday, excluding holidays and any day declared an official federal holiday. Holidays observed by the VA Medical Center are: New Year s Day Labor Day Martin Luther King Day Columbus Day President s Day Veteran s Day Memorial Day Thanksgiving Day Independence Day Christmas Day If a government holiday falls on a Monday, then scheduled delivery will be on Tuesday of that week. If a government holiday falls on a Wednesday, then scheduled delivery will be on Tuesday of that week. If a government holiday falls on a Friday then scheduled delivery will be on Thursday of that week. VA has the option to increase, decrease, or cancel any delivery. Contractor shall receive one (1) day advance notice in any of these situations. Safety requirements: In the performance of this contract, the contractor shall take such safety precautions as the Contracting Officer or his designee may determine it be reasonably necessary to protect the lives and health of occupants of the building. B.4 LIMITATIONS ON SUBCONTRACTING-- MONITORING AND COMPLIANCE (JUN 2011) This solicitation includes FAR 52.219-4 Notice of Price Evaluation Preference for HubZone Small Business Concerns. Accordingly, any contract resulting from this solicitation will include this clause. The contractor is advised in performing contract administration functions, the CO may use the services of a support contractor(s) retained by VA to assist in assessing the contractor's compliance with the limitations on subcontracting or percentage of work performance requirements specified in the clause. To that end, the support contractor(s) may require access to contractor's offices where the contractor's business records or other proprietary data are retained and to review such business records regarding the contractor's compliance with this requirement. All support contractors conducting this review on behalf of VA will be required to sign an Information Protection and Non-Disclosure and Disclosure of Conflicts of Interest Agreement to ensure the contractor's business records or other proprietary data reviewed or obtained in the course of assisting the CO in assessing the contractor for compliance are protected to ensure information or data is not improperly disclosed or other impropriety occurs. Furthermore, if VA determines any services the support contractor(s) will perform in assessing compliance are advisory and assistance services as defined in FAR 2.101, Definitions, the support contractor(s) must also enter into an agreement with the contractor to protect proprietary information as required by FAR 9.505-4, obtaining access to proprietary information, paragraph (b). The contractor is required to cooperate fully and make available any records as may be required to enable the CO to assess the contractor's compliance with the limitations on subcontracting or percentage of work performance requirement. B.5 SUBCONTRACTING COMMITMENTS--MONITORING AND COMPLIANCE (JUN 2011) This solicitation includes VAAR 852.215-70, Service-Disabled Veteran-Owned and Veteran-Owned Small Business Evaluation Factors, and VAAR 852.215-71, Evaluation Factor Commitments. Accordingly, any contract resulting from this solicitation will include these clauses. The contractor is advised in performing contract administration functions, the CO may use the services of a support contractor(s) to assist in assessing contractor compliance with the subcontracting commitments incorporated into the contract. To that end, the support contractor(s) may require access to the contractor's business records or other proprietary data to review such business records regarding contract compliance with this requirement. All support contractors conducting this review on behalf of VA will be required to sign an Information Protection and Non-Disclosure and Disclosure of Conflicts of Interest Agreement to ensure the contractor's business records or other proprietary data reviewed or obtained in the course of assisting the CO in assessing the contractor for compliance are protected to ensure information or data is not improperly disclosed or other impropriety occurs.
