Attachment 1- CHP-16010-6J&DPW-17139-7J SOW 30 Aug 18 W912D018Q0067 CHP-16010-6J Relocate Sign and Sidewalk Repair Solicitation SF 1442 7 Sept 18 ***THE ENTIRETY OF THE PROJECT IS TO BE COMPLETED IN INTERIOR ALASKA*** The U.S. Army Garrison at Fort Wainwright has a requirement to relocate a electronic sign and repair, replace concrete sidewalks at building 4107, Southern Cross Chapel. The contractor shall provide all labor,fittings, equipment, components, mounts, devices, accessories, transportation,and storage as required to perform all work in accordance with specifications,commercial practices and industry standards. See statement of work (SOW) and attachments for further information. A site visit will be conducted on 11 Sep 2018 at 10:00 AM Alaska Time (AKT)at BLDG 3015, Department of Public Works (DPW), Facilities Management Conference Room, Fort Wainwright, Alaska. Failure to attend the site visit in accordance with FAR Clause 52.236-27 to identify obvious site conditions does not constitute a valid claim for differing or unforeseen site conditions. Any questions must be submitted by e-mail to remodino.s.delacruz.civ@mail.milno later than 12 Sep 2018 by 2:30 PM (AKT). Quotes are due by 18 September 2018 at 2:30 PM AKT. Quotes must be receivedelectronically by remodino.s.delacruz.civ@mail.mil No other method ofquote submission will be accepted. Adobe (.pdf) and Microsoft (.doc) are acceptable formats for submitting quotes. The electronic file must be under 8MB total to ensure it can be received by e-mail. If you are having any issues submitting your quotes, please contact Remo DeLaCruz as soon as possible. **Please submit quotes/offers on attached SF 1442** The magnitude of this project is less than $25,000.00. The Government reserves the right to cancel this solicitation. Any questionsshall be sent to remodino.s.delacruz.civ@mail.mil **INSTRUCTIONS TO OFFERORS** (a) Submitting your quote. Submit/fill in offeror amount/company's quote on the SF 1442, page 3, section B electronically to remodino.s.delacruz.civ@mail.mil. You may submit more than one quote. If you do, we will evaluate each separately. (b) Submission deadline. We must receive your quote at the e-mail address above by SF 1442, 18 Sep 2018, 2:30 PM AKT. We will not consider any quote that we receive after the deadline. (c) Terms and content of your quote. Your quote must be based on the terms of this request for quote (RFQ). We may reject any quote that is not based on these terms in every respect. (d) Evaluation procedures. The Contracting Officer will consider past performance as well as price when making a decision for award. Award may be made to other than the lowest priced offeror if upon review of past performance information it is determined to be in the best interests of the Government. FAR 13.106-2(b) allows for the evaluation of price and other factors to include a review of PPIRS or any other reasonable basis which includes a FAPIIS, SAM, & EPLS review. All eligible offerors are evaluated on Lowest Price after a PPIRS, FAPIIS, SAM, and EPLS review to exclude those with negative past performance. (e) Site visit: Failure to attend the site visit listed in FAR Clause 52.236-27 and identify obvious conditions does not constitute a valid claim for a differing or unforeseen site condition.
