- Statement of Need: Requesting location: Richard L. Roudebush VA Medical Center, 1481 West 10th Street, Indianapolis, IN 46202 Background Information: Due to broken plumbing pipes, the Nutrition Space in A-8116 in Building # 01, West Tenth Street, Indianapolis, IN is in need of new base cabinets. Base cabinets made up of standard construction (MDF or plywood core construction material) will not meet the need of this space were a leak from the pipes to occur again. Due to coordination in plan with the arrangement of the upper cabinets (that are remaining in the space), the existing plumbing rough-in, and the placement of auxiliary equipment necessary for this space (refrigerator, ice maker, and coffee maker), the casework must be sized as noted below (not standard casework size): Scope: The following criteria must be met in the final selection: Cabinetry common to room (A-8116) Overall lower casework size to be no more than 24.0 deep x 34.5 in height; widths of individual cabinets will vary to conform with the space/plan. Base cabinets to be constructed of powedercoated 18 gauge cold rolled steel, finished on all four sides with leveling screws on each corner. Front of base cabinets; doors and drawers finish/colour: to closely match existing upper cabinets (Midmark brand Radiance or identical match). Base cabinets to consist of single drawer atop door compartment with the exception of the corner base (full-height door). Drawers to be one piece molded polystyrene body with rounded corners; drawer-front to be 4.5 in height. Door/Drawer fronts to have 12 mil seamless vinyl wrap. finish/colour: to closely match existing upper cabinets (Midmark brand Radiance or identical match). Hinges to be Blum concealed, self-closing, 110 degree opening, nickel plated metal, clip-on mount, three adjustable points. Door and Drawer handles to be brushed nickel pulls. Style: to closely match existing upper cabinets. Page 1 of 5 Scope (con t): Counter-top with integrated, single-bin sink for above stipulated cabinets (A-8116) Countertop to be solid-surface acrylic with smooth, fused seams, set-on back/side splash, and single integrated sink basin. Counter-top front and side edges to be free of sharp edges. Counter-top to cover bsae cabinets and pipe-chase created at the back of the cabinets (during installation, defer to plan) and have a 1.5 overhang to the front edge of the cabinets. Counter-top colour/pattern is to be similar to (if not actually) Corian Antarctica to coordinate/harmonize with existing flooring in space Holes to be bored into counter-top material for placement of faucet at the sink and for water-lines to deliver water to the coffee-maker and ice-maker. Faucet for above stipulated counter-top with integrated sink (A-8116) Chrome finish, gooseneck style with wrist blade handle(s). Plan of space: Page 2 of 5 Warranty: Lifetime warranty against defects of materials and workmanship under normal use and service. Minimum 5 year warranty. Contractor Requirement: Field Verification: Contractor will field verify installation locations prior to order being processed to ensure proposed unit will work in each location. Warranty: The products will be free from defects in workmanship and materials for a period of five years. Packaging, Packing, and Shipping Instructions: The casework shall be delivered to installing vendor and installer will deliver to Richard L. Roudebush VA Medical Center, 1481 West 10th Street, Indianapolis, IN 46202 during normal business hours. Installation will be coordinated with COR John S. Bendel, Engineer Services Interior Designer. Inspection and Acceptance Criteria: The COR will inspect the product upon receipt, oversee installation and notify the contractor of any defects or broken parts. Contractor is responsible for the cost of returns, replacement parts and labor for repairs identified upon receipt. Submittals: must be reviewed by COR/Engineer Services Interior Designer before final selection is made. The C & A requirements do not apply; a Security Accredation Package is not required. Submit price list for casework to include breakdown of cost for optional features requested. Submit material samples for finish options. Submit a direct comparison to the product specified for each proposed alternate. Submit all warranties and maintenance manuals to VA. Period of Performance: The shipment date shall be no later than 6 weeks after bid has been awarded. Expected delivery date: No later than September 01st, 2018. Delivery, Storage and Handling of Materials. Product shall be factory packed with adequate protection to all sides and corners to prevent transit and handling damage. All packaging to be removed from premises by installing vendor. Installation will be coordinated with COR, John S. Bendel, Engineering Services, Interior Designer.
