- Sources Sought Notice- 36C24418Q9791 The intent of this Sources Sought Notice is to identify potential any businesses, particularly that of Small Businesses especially any SDVOSB offerors capable of providing a Visera 4K UHD as listed in the Statement of Need (See Below). These are for the VA Healthcare Center in PHILADELPHIA, PA 19104. Responses to this Sources Sought Notice should demonstrate the firm's ability, capability, and responsibility to provide the principal components of supplies listed in the Statement of Need (See Below). Responses should include the following information: Business name Address Business point of Contact Current Duns Number All information is to be submitted via e-mail at Amanda.saunders@va.gov Information provided will not be returned. All responses shall be in the English Language. Responses are due by 10:00 am (EST) on 9/13/17. STATEMENT OF WORK (SOW) GENERAL INFORMATION Title of Project Olympus Visera 4K UHD Scope of Work Contractor shall furnish including but not limited to all labor, materials, parts, equipment, tools, transportation, and supervision to provide the Corporal Michael J.Crescenz VA Medical Center in Philadelphia PA. The Contractor shall provide all necessary services including setup, configuration, database transfer and installation to get the specified equipment working properly. The Contractor shall provide all necessary training to the designated VA employees on how to use the installed equipment after completion of installation. The equipment, all of its accessories, and supplies that come with the equipment shall be brand new. The equipment, all necessary services related to removal, mobile unit usage, setup, configuration, installation, and training services are hereinafter collectively referred as Deliverables. All Deliverables shall be delivered based on an agreed schedule of work. Vendor will agree to provide trade-in discount for existing Olympus equipment in ENT clinic. Background The Visera 4K System will allow other providers and residents to visualize all the anatomies and abnormalities simultaneously while staff physician is performing the scope examination. This enhances patient care, avoid duplicate examination by the team members thus minimize patient discomfort. It also can be viewed during patient chart rounds, physician peer review, etc. Thus will improve our patient care quality assurance. Extra low dispersion glass lenses allows for sharp images High contrast at high spatial frequencies and improved color display. ENF-VH RHINO-LARYNGO Video Scope: The ENF-VH has a viewing angle of 110 degrees. And with HD resolution and exceptional illumination, every corner of the field can be thoroughly inspected. ENF-VH delivers HD images, which show greater detail for examinations Pre-Freeze function continuously captures still images even before you push the Pre Freeze button. When you press the button, the system automatically selects the sharpest image among several NBI (Narrow Band Imaging): NBI is a patented optical image technology that enhances the visibility of vessels and other tissue on the mucosal surface. NBI works by filtering the white light into specific light wavelengths, which are absorbed by hemoglobin and penetrate only the surface of human tissue. This highlights areas of increased vascularity which are normally difficult to distinguish. PRODUCT SPECIFICATIONS AND FEATURES The system, all of its accessories, auxiliaries, parts and supplies shall be brand new (No refurbished products will be accepted). DELIVERY, WORK HOURS, AND COORDINATION FOR SETUP AND TRAINING SERVICES Delivery Timeframe: All Deliverables shall be completed based on a schedule of work provided after award, preferably within one month of Contractor s receipt of a delivery task order from the Contracting Officer. Shipping and Handling: Contractor shall be responsible for the shipping and handling of all the equipment. Work Hours All work and delivery shall take place during clinical hours (Monday through Friday, 8:00 am to 4:30 pm excluding weekends and federal holidays). Federal holidays include the following: New Year s Day, Washington s Birthday, Martin Luther King s Birthday, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving, Christmas, and any days declared by the Government as holidays. Installation Contractor shall provide all labor, materials, tools, equipment and etc. to prepare for the installation of the equipment at the CVAMC This equipment will be installed for ENT Clinic All employees of the general contractor and subcontractors shall comply with CVAMC policies including but not limited to: vendor credentialing, parking, check-in procedures, name badges, smoking policies and confidentiality. Contractor is responsible for removing all packing materials and related waste and dispose of off-site TESTING AND CERTIFICATION Upon completing installation of the system, the Contractor shall notify the ENT Staff in writing that the system is ready for acceptance testing, and that it meets all requirements as specified. Submission of this notification of system readiness shall be accomplished by the contractor prior to the beginning of the scheduled Government Acceptance Test. EQUIPMENT MANUALS The Contractor shall furnish User Manual and reference guides for the specified equipment. PRODUCT MODIFICATION, REMOVAL OR RECALL: If any product awarded under this solicitation requires modification, is removed or recalled by the Contractor or manufacturer, or if any required modification, removal or recall is suggested or mandated by a regulatory or official agency, the following steps will immediately be taken by the Contractor or manufacturer: Notify the Contracting Officer, in writing, by the most expeditious manner possible. Provide two copies of the notification which shall include, but not be limited to the following: Complete item description and/or identification, order numbers from customers, and the contract number assigned as a result of an award on this solicitation. Reasons for modifications, removal or recall. Necessary instructions for return for credit, replacement or corrective action. b. Provide the above information to all agencies and VA Facilities who purchased the product. The Contracting Officer shall be provided a copy of the notification, and a list of all agencies and/or VA facilities notified c. Contractor shall be responsible for any costs associated with the above.
