- SF-18 Scope of WORK RFQ ADMIN INFO Solicitation Provisions ***************** PLEASE READ THIS COMPLETE SYNOPSIS *************** This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Quotes are being requested and a written solicitation will not be issued. The current solicitation number is 18-239-SOL-00120 and is issued as a Request for Quotation (RFQ). This acquisition is being conducted under Simplified Acquisitions Procedures in accordance with FAR part 13. The clauses and provisions referenced in this solicitation may be obtained in full text form at http://www.arnet.gov/far. The Department of Health and Human Services/Indian Health Service has a requirement for a vendor to be a host providing Policy Manager and Learning Management System. for the Red Lake Hospital. Please read the Scope of Work (SOW) for more information. Please see attachment entitled LMS SOW for additional information. Please send questions via e-mail to Christopher.Millard@ihs.gov. I will submit the questions to the requestor. He will then answer them. I will then do an amemdment to the solicitation and post as Q & A. We are wanting to do a one (1) year Firm Fixed Price contract. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-66. The associated North American Industrial Classification System (NAICS) code for this procurement is 541519 with a small business size standard of $27.50 Million Dollars. Set Aside: This requirement is set as a 100% small business set aside. BASIS FOR AWARD The award will be based on the following Evaluation Factors: 1) Price Reasonableness 40% 2) Past Performance and Experience 30% 3) Ability to Meet the SOW 30% Please read all information on SF-18 which is attached The following attachments are posted to this solicitation: 1) Solicitation Provisions 2) RFQ entitled LMS RFQ. Please fill out page 1 Contractor Name and Address as well as TIN, DUNS, Phone, and E-mail. Also, please fillout page 4 pricing. Please quote monthly or quarterly as this is preferred way for the Governemnt. If your company doesn't invoice like this we can have you do it annually as a last resort. Please send with your propsal 3) Scope of Work (SOW) 4) SF-18: Please fill in block 8, 12 (if applicable), 13, 14, 15, and 16 and send with your proposal Please send your proposal package via e-mail to Christopher.Millard@ihs.gov In your proposal package please send a business quote. Failure to send a business quote the vendor will be considered unresponsive. The pricing sheet on the RFQ in number 2 above is not considered a business quote.
