Description
The purpose of this solicitation is to procure Annual Support and Maintenance for 1 each Candela Vbeam Perfecta Laser System, base year plus three (3) option years in support of Martin Army Community Hospital, Fort Benning, Georgia. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation number W91YTZ-18-Q-0223 is issued as a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-100 and Defense Federal Acquisition Supplement Publication DPN 20171228 current as of 28 December 2017. This acquisition is issued on a 100% Small Business Set-Aside basis under NAICS code 334510 and size standard is 500 employees. All responsible sources may submit a quotation which shall be considered by the agency. The purpose of this solicitation is to procure Annual Support and Maintenance for 1 each Candela Vbeam Perfecta Laser System, base year plus three (3) option years in support of Martin Army Community Hospital, Fort Benning, Georgia. Period of Performance: 1 October 2018 through 30 September 2022, subject to 52.232-18 -- Availability of Funds. Quotes are due by 25 September 2018 at 12:00 pm EST. Questions must be submitted via email no later than 21 September 2018 at 12:00 pm EST. Quotes and questions shall be submitted via email to linda.m.spindler.civ@mail.mil. The contractor shall provide written evidence that service personnel to be utilized on this contract shall have either been trained by the Original Equipment Manufacturer (OEM) on the specific equipment they service and experience within the past three years or have been certified as a Certified Biomedical Equipment Technician by the Association for the Advancement of Medical Instruments (AAMI) and have a minimum experience within the past three years or have an Associate Degree in Biomedical Technology or Electronics with a minimum of 3 years' experience servicing the specific equipment.
