Description
THIS IS A SOURCES SOUGHT NOTICE ONLY. This notice does NOT constitute a request for proposal, request for quote, or invitation for bid. No formal solicitation document exists at this time. Market Research is being conducted to Service Disabled Veteran Owned Small Business (SDVOSB), Veteran Owned Small Business (VOSB), Small Business, Small Disadvantaged Business, 8(a), Woman Owned Small Business or HUBZone interests. The Hampton Veterans Affairs Medical Center (VAMC) is trying to determine whether to set-aside an acquisition to provide Annual UV Light Preventive Maintenance contact with the Asheville VAMC in Ashville North Carolina. The scope of the acquisition includes but is not limited to: Vendor will provide service maintenance for UV lights including scheduled preventive maintenance and unscheduled corrective maintenance on four UV lights located at VAMC Asheville. Preventive maintenance as well as software maintenance and updated will be scheduled in advance during the months they are due and performed during normal working hours between 8 am and 5 pm. Product training and Q&A upon request on site 2 day/year (up to 6 hours per day) and /or on- line web-cast 4 times a year. Software updates for software on the R-D machine, usually 2-4 times per year or as needed. 4G network access coverage. Online data access and reporting includes operation, utilization, trigger e-males, utilization e- mail account review to help drive system utilization. Routine inspections as needed. Repairs and maintenance to include all parts and labor for all system component repairs (lamps, ballasts, computers, sensors, batteries, etc.). Sensor and recalibration will be conducted once (1) a year (4 UVC sensors): to be completed upon service agreement renewal annually or as needed. The preventive maintenance will be performed in accordance with manufacture s recommended procedure as stated in the Sterrad service manuals. After each repair and scheduled maintenance visit, the Contractor shall provide a written service report indicating the date of service, the model, serial number and location of equipment serviced, the name of the service representative, the service performed, and the parts replaced. The contractor shall furnish and replace all necessary parts as required to return equipment to full manufactures published specifications. The contractor shall use new parts only. The reports shall be delivered to the Assistant Chief, Environmental Services, Building 47, Room # BF-132. If your company is interested in performing these services, please notify our office of your intent. In response, please include the following: (a) a positive statement of your intent to submit a bid as a prime contractor (capability statement); (b) evidence of experience in work similar in type and scope to include contract numbers, project titles, dollar amounts, points of contact and telephone numbers; (c) past performance references with points of contact and phone numbers. At least two (2) references are requested, but more are desirable. Responses to this notice should also identify whether or not the firm falls into any of the socioeconomic categories listed above. Your response is required by: 25 September 2018, 4:30 PM All capable Service Disabled Veteran Small Businesses are encouraged to respond. The Government will not pay for any information solicited. All of the above must be submitted in sufficient detail via email to: warren.pennil@va.gov
