Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. The proposed contract action is for supplies or services for which the Government intends to solicit and negotiate with only one source under the authority of FAR 13.106(b)(1). Interested persons may identify their interest and capability to respond to the requirement or submit proposals. This notice of intent is not a request for competitive proposals. However, all proposals received prior to 09/19/17 will be considered by the Government. A determination by the Government not to compete with this proposed contract based upon responses to this notice is solely within the discretion of the Government. Information received will normally be considered solely for the purpose of determining whether to conduct a competitive procurement. SUPPLIES/SVCS PRICE AMOUNT 055WH380040 1 EA $ $ MOBILE MEDICAL UNIT ____________________ GRAND TOTAL --- ==================== Salient Characteristics: THIS VEHICLE MUST BE CUSTOMIZED TO BE COMPATIBLE AND ABLE TO INTEGRATE WITH EXISTING FARBER SPECIALTY VEHICLES CUSTOM BUILT FOR THE VA THAT ARE CURRENTLY IN USE IN THE FIELD. THIS IS A SPECIALIZED VEHICLE USED FOR VETERAN HEALTHCARE. VEHICLE MUST CONTAIN THE FOLLOWING: 2 exam rooms Reception/triage area Bathroom with sink and toilet Sinks in exam rooms Vitals machines in exam rooms and triage area Ophthalmoscope in exam rooms and triage area Desks in each exam room and smaller desk in triage area, 3 chairs and stools Monitors in exam rooms above desks- for telehealth- with adjustable arm Cabinets/drawers for files and additional for supplies Storage underneath van Area for coffee machine/sm microwave/fridge 1 diesel generator- shared tank for engine and power for inside van (lights, heating/ac, etc) Wheelchair lift- prefer on back of vehicle Awning Slide outs for both exam tables and pop out rooms extensions (see attached picture) Can be driven with regular Class C license, not commercial Each area of vehicle will be controlled by separate thermostat Electronics, audio, video, intercom Three-camera backup monitor system with dash mounted display Backup sensors to detect object(s) near rear of vehicle Overhead obstruction sensor to prevent running into items with low clearance DVD player with audio/video switch so that output may be routed to any, all, or part of the 3 LED TV/Monitors. Switch should allow for 2 additional sources to be connected; such as satellite TV receivers or computers. Connections from switch to TV/Monitors will be HDMI. VTC & TV locations will be provided with, HDMI and 3 wire video, sound connections, electrical and ethernet Provide two 32 inch LED TV/Monitors, one to be installed in front area and one in rear area on a swing arm that allows viewing from different angles Provide one exterior 32 inch LED TV/Monitor (mounted within a weather proof compartment located on the passenger side of vehicle, under the awning). This compartment will open to allow monitor to be seen and not interfere with awning or doors operation. TV should have sound routed through public address system to provide amplification. Exterior network connections will be located within a weather proof lockable compartment Provide a roof mounted HD TV antenna, low profile, not required to be raised. Provide four GFI outlets; four POE connections; and two video connections and one public address system microphone connection; within a locked compartment under the awning area. These may be located within the TV compartment. Provide an integrated public address system. There should be at least one microphone and one aux input connections inside of electronics cabinet. One exterior microphone plug co-located with other exterior connections (all exterior connections will be installed within a lockable compartment). Speakers for the system should be of an all-weather proof design and permanently mounted on vehicle exterior. External TV will need connected through the public address system. Provide a secure weather tight electronics cabinet; accessible from both interior and exterior of vehicle Within the electronics cabinet provide and install a 10U Rack that may be pulled out and accessed through outside door for working on components An UPS (uninterruptable power supply) will be provided and mounted in the rack (2U; 120 v in & out; 1500 VA; 1200W; sine wave) A patch panel connecting all POE outlets and phone cables will be mounted within the electronics cabinet All network cabling will be CAT 6 with connections & cable wired for POE Provide, install, and connect a HP Color LaserJet Pro MFP M477fnw Replicate existing vehicles within the VA fleet to improve and maintain interoperability across VISN lines and improve emergency response capability Delivery, acceptance and F.O.B Destination (freight prepaid) point: VA Palo Alto Healthcare System 3801 Miranda Avenue Palo Alto, CA 94304 Period of Performance: 180 DARO Only electronic offers will be accepted.
