STATEMENT OF WORK (SOW) Centricity Cardiology DMS Software Contract Number: 36C25018P4813 Task Order Number: IFCAP Tracking Number: 539C85213 Follow-on to Contract and Task Order Number: 1. Contracting Officer s Representative (COR). Name: Steve Campanello Section: Medical Service Address: 3200 Vine Street. Cincinnati, OH 45220 Phone Number: 513-475-6358 Fax Number: 513-475-6399 E-Mail Address: Steven.campanello@va.gov 2. Contract Title. GE Maclab 4.3 DMS upgrade 3. Background. There is a need for Electronic Medical Record interface with Mac Lab Upgrade already in NAC 4. Scope. The procurement of his software is limited to the Cincinnati VA Medical Center Cardiac Cath Section. 5. Specific Tasks. Centricity Cardiology DMS is intended to provide access to patients complete cardiovascular electronic medical record, integrating information and data from electrophysiology, cath, nuclear, echo, and diagnostic ECG. This release contains no requirements which significantly change the user needs or intended uses of this product. 6. Performance Monitoring Not applicable 7. Security Requirements The C&A requirements do not apply and a Security Accreditation is not required. 8. Government-Furnished Equipment (GFE)/Government-Furnished Information (GFI). 9. Other Pertinent Information or Special Considerations. Inspection and Acceptance Criteria. Centricity Cardiology DMS software will be delivered through the warehouse. The item manager for the Cath Lab will deliver to the Cath Lab. 10. Risk Control Not applicable 11. Place of Performance. This work will be performed at the Cincinnati VAMC 3200 Vine Street Cincinnati OH 45220 12. Period of Performance. 09/27/2018 01/31/2019. Installation shall be on the coordinated scheduled date between COR and vendor. 13. Delivery Schedule. Coordinated installation date with vendor for install.
