Description
Amendment to the Notice of Intent to Sole Source: Suicide Prevention Database The Department of Veterans SAO West, Network Contracting Office 19, located at 6162 South Willow Drive Suite 300, Greenwood Village, CO 80111 intends to enter into a sole source, base plus four (4) Option year order authorized by FAR 13.106-1(b), Only One Responsible Source and No Other Supplies or Services Will Satisfy Agency Requirements. Under the authority of FAR 13.106-1(b), the Department of Veterans Affairs intends to award a sole source purchase order to the small business, Previdence Corporation located at 5685 South 1475 East Suite B, Ogden, UT 84403. The associated North America Classification Systems (NAICS) Code is 541511 and the Business Size Standard is $27.5 million. The contractor shall provide the George E Whalen VA Medical Center in Salt Lake City, UT, with the Suicide Prevention Database required for online customer support 24/7, entering vital information relating to patients who are deemed high- risk based on their behavior and tracking patients for follow up calls, letters, appointments. The VA, specifically in the Veterans Integrated Services Networks 19 (VISN) locale has been using Previdence s suicide prevention program database since 2009. Utilizing the current system eliminates substantial duplication of cost to the Government that is not expected to be recovered through competition in fulfilling the agencies requirements. It is VHA policy to standardize to the maximum extent possible the types and kinds of supplies and equipment it purchases, consistent with clinical and practitioner needs. Utilizing Previdence s suicide prevention database allows for standardization expected to facilitate best-value product pricing, facilitation and delivery of high-quality health care to veterans. This notice of intent is not a request for competitive proposals or quotes. Interested parties may identify their interest and capability to respond to the requirement. A determination not to compete this requirement, based upon responses to this notice, is solely within the discretion of the U.S. Government. Interested persons may identify their interests and capability to respond to this requirement no later than 2:00 PM Eastern Standard Time, Wednesday October 24th, 2018. Any response to this notice must show clear and convincing evidence that competition would be advantageous to the Government in future procurements. Supporting evidence must be furnished in sufficient detail to demonstrate the ability to meet the subject requirement. This information shall be provided at no cost or obligation to the Government. The following option clauses have been added 52.217-8 OPTION TO EXTEND SERVICES (NOV 1999) The Government may require continued performance of any services within the limits and at the rates specified in the contract. These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months. The Contracting Officer may exercise the option by written notice to the Contractor within 30 calendar days. (End of Clause) 52.217-9 OPTION TO EXTEND THE TERM OF THE CONTRACT (MAR 2000) (a) The Government may extend the term of this contract by written notice to the Contractor within 30 calendar days; provided that the Government gives the Contractor a preliminary written notice of its intent to extend at least 60 calendar days before the contract expires. The preliminary notice does not commit the Government to an extension. (b) If the Government exercises this option, the extended contract shall be considered to include this option clause. (c) The total duration of this contract, including the exercise of any options under this clause, shall not exceed 60 months.
