Solicitation 36C25219Q0059 Notice of Intent to Sole Source reference FAR 6.302-1. The Great Lakes Acquisition Center is procuring the following, for the Edward Hines Jr. VAH in Hines, Illinois: Brand name (URETRON) only lithotripter to include: Quantity Item Number Description 1 URT-F-US URETRON FULL SYSTEM, BNDL 1 URT-G-US URETRON GENERATOR 1 URT-FS-D URETRON FOOTSWITCH DUAL CONNECTOR 2 URT-TR URETRON RIGID HAND PIECE 2 URT-W URETRON WRENCH 1 URT-CR URETRON CLEANING ROD 1 URT-PC-US URETRON POWER CORD 3 URT-PR-3.5-370S URETRON RIGID STERILE PROBE 3.5X370 The Great Lakes Acquisition Center intends to award on a sole source basis i/a/w: VAAR 819.7007, a purchase order to ERA HEALTH, LLC (dba VETERANS HEALTH MEDICAL SUPPLY), 777 Main Street, Suite 770, Fort Worth, TX 76102-2521, as the only responsible source to provide the above items. The statutory authority for other than full and open competition is VAAR 819.7007. The applicable NAICS Code is 334510 and the size standard is 1,250 employees. Interested persons may identify their interest and capability to respond to the requirement or submit quotes. This notice of intent is not a request for quotation or a solicitation of offers. However, all quotes received by 10:00 AM on November 01, 2018 will be considered by the Government. A determination by the Government not to compete this requirement upon receipt of response to this notice is solely within the discretion of the Government. There will be no solicitation available.
