DEPARTMENT OF HEALTH AND HUMAN SERVICES Centers for Disease Control and Prevention (CDC) National Center for Health Statistics (NCHS) Request for Information (RFI) No. 2019-RFI-NHCS National Hospital Care Survey - NHCS Project Title Request for information on United States hospital databases that gather electronic health record (EHR) data that can be used to supplement data collected by the National Hospital Care Survey (NHCS). The Government will accept questions regarding this RFI. All questions will be reviewed and answered as decided by the Government. Please ensure that any e-mail responses have "2019-RFI-NHCS" in the subject line. Any responses received after the above response due date and time will not be accepted or answered. Introduction This is a request for information only to obtain information about EHR data collected for United States hospitals. It is not a request for proposal and does not commit the Government to issue a solicitation, make an award, or pay any costs associated with responding to this announcement. All submitted information shall remain with the Government and will not be returned. The objectives of this RFI are to: 1. Gather information on vendors who collect and maintain EHR data for a large number of U.S. hospitals. Information on databases that collect administrative claims data is not sought. 2. Obtain information on the procedures and methods used to gather hospital EHR data. 3. Attain an approximate estimate of the cost of purchasing hospital databases with EHR data. 4. Optional and at the discretion of the Government: invite vendors to present information about their hospital databases in person, onsite at the Centers for Disease Control and Prevention (CDC)/National Center for Health Statistics (NCHS). Background The Centers for Disease Control and Prevention (CDC), National Center for Health Statistics (NCHS) conducts the National Hospital Care Survey (NHCS), which is designed to describe national patterns of health care delivery in hospital-based settings, including inpatient departments, emergency departments (EDs), and outpatient departments (OPDs), along with hospital-based ambulatory surgery locations. Data on patients and their care are collected from a sample of hospitals to provide national estimates of service use. The current sample consists of 598 nonfederal and noninstitutional hospitals with six or more staffed inpatient beds. Sampled hospitals are requested to transmit all inpatient, ED, and OPD encounters for a 12-month period from January to December. Objective and timely data are essential to assess the health and well-being of the in-care population and the performance and functioning of the health care system. When fully implemented, NHCS aims to allow healthcare decision makers, policy makers, and researchers to track the latest trends affecting hospitals and health care organizations, to study relationships between the organization and delivery of health care, and to benchmark hospitals against national data on a variety of measures. Additionally, NHCS collects patient-level identifiers that allow a patient's episodes of care to be linked between different hospital inpatient and ambulatory settings, as well as to outside databases such as the National Death Index and Medicare data, providing a more complete picture of patient care. Initially, NHCS began collecting Uniform Bill (UB)-04 administrative claims data from sampled hospitals. However, in 2015, NHCS began transitioning to collection of EHR data, which affords much more clinical information (e.g., laboratory tests ordered and results, diagnostic ordered and results, medications and clinical notes) than administrative claims data. In 2016 and 2018, NHCS data collected are a mix of administrative claims and EHR data. To decrease participation burden, NCHS is exploring the option to gather more encounter level EHR data for NHCS sampled hospitals through third party sources. Information such as how hospitals are selected, entered, organized, and maintained in the database is important to understand. Just as important as the breadth of the database is the type of information collected about each hospital and the manner in which it is collected, organized, processed and updated. Appendix A includes a list of variables that are of interest for analysis. In addition to information about the variables that are collected, information about how data are collected (e.g., primary or secondary data collection), in what format are the data stored and organized, the date of collection, how much data processing is performed, and how often data are updated to ensure accuracy is central to NCHS in order to provide timely and reliable national health care utilization estimates. Information requested We expect a responder to address the following areas: 1. Overall Corporate Capability Provide a brief Statement of Corporate Capability (SOCC) on your company and include a client list of Government agencies and private companies that you have supported. The SOCC and client list are limited to 1 page, double-spaced, in Times New Roman 12 font, and 1" margins. 2. Hospital Data Experience Provide a brief statement of your specific experience with collecting, organizing, and maintaining hospital EHR encounter level data including (but not limited to): eligibility rules for inclusion in the database, most recent year of data, frequency of data collection/updates (e.g., annually or biannually), level of data collection (e.g., from the hospital, hospital network, etc.), and total number of hospitals in the most recent version of the database. Written responses are limited to 3 pages, double-spaced, in Times New Roman 12 font, and 1" margins. 3. Capabilities in Variable Collection CDC/NCHS is seeking information on the capabilities of organizations collecting hospital EHR encounter level data to provide information about the variables listed in Appendix A. Submissions should cover ability to collect each variable described that is required for creating national estimates (marked with a "Y") and ability to collect any other variables of interest (marked with a "N"). Written responses to questions regarding capabilities are limited to 3 pages, double-spaced, in Times New Roman 12 font, and 1" margins. 4. Data Dictionary CDC/NCHS is seeking information on the hospital database contents, format, and structure, as well as the relationship among its elements, including variable values and missingness, for the variables required and of interest listed in Appendix A. Written responses to questions regarding capabilities are limited to 3 pages, double-spaced, in Times New Roman 12 font, and 1" margins 5. Cost CDC/NCHS is seeking an approximate cost estimate for the U.S. hospital databases in their entirety and for only the variables listed in Appendix A for budgeting purposes in the event the Government wants to purchase the data. Written responses to costs are limited to 1 page, double-spaced, in Times New Roman 12 font, and 1" margins.
