This is a SOURCES SOUGHT announcement only. It is neither a solicitation announcement nor a request for proposal or quote and does no obligate the Government to award a contract. Requests for a solicitation will not receive a response. Responses to this Sources Sought must be in writing. The purpose of this Sources Sought Announcement is for market research to make appropriate acquisition decisions and to gain knowledge of potential qualified businesses. Responses to this notice shall include: Company Name Address Point of Contact Phone, Fax, and Email DUNS number Cage Code Type of business, e.g. Service Disabled Veteran Owned Small Business, Veteran Owned Small Business, 8(a), HUB Zone, Women Owned Small Business, SDB Must provide a capability statement that addresses the organizations qualifications and ability to perform the work described below: The Harry S. Truman Memorial Veteran s Hospital in Columbia, MO is looking to procure Eye Lane Equipment for the Jefferson City CBOC s Surgery Service/Eye department. This procurement will allow for standardization within the exam rooms that allow for appropriate and timely Eye care to be provided to our Veteran patients. Three complete eye lane systems to be installed and functional at; Jefferson City CBOC The Eye clinic lanes need to be a system that works completely independent and provides all the necessary exam capabilities to perform complete complex Ophthalmology and Optometry eye examinations for patient with any handicap or disability. The eye lane should include at minimum the following: Ophthalmology patient chair the is wheelchair accessible by the patient and easy moved and cleaned for staff. The chair should be easy cleaning and infection control within any clinical environment. The chair should be powered with foot pedal operations. The stand and light source must be of medical standards for cleaning capability to fulfill standards set by joint commission. This stand must have a minimum of 3 charging stations that would hold a transilluminator, direct ophthalmoscope, and a retina scope. The stand must have a movable arm that would hold a table that will fit a standard slit lamp that moves from a resting position away from the patient to a position that would line up with center of the patient chair. The stand should have a moveable are that has a dimmable room light. It must also have adjustable arm that hold phoropter. Visual Acuity computer station that is wall mounted and can be calibrated accurately for just 3 feet from patient. This Visual Actuality must have approved software by JCOAHP and American optometric association. The software and testing must meet standard of care for American optometric society and be standard or above standard testing. It must have standard VA testing, Ichihara color plate testing, ability to upload personal patient care information and have standard patient education already uploaded to the system and be remote controlled. The LED slit lamp that mounts on the table on the stand, have a teaching tube, have ability to gonio and check tonometry. Pneumatic stool Tonometer: Pen and applanation. Applanation slit lamp attachment with disposable covers for pen and single use options for applanation. Phoropter in minus cylinder black that has ability to mount to eye stand described above. 101 lens meters Retinoscope, Transilluminator, and ophthalmoscope that must be all able to be recharged in charging wells described in above stand. 500LEDHQ indirect chargeable Headgear with wall mount charger. Trial Lens set that has case and easy display in minus cylinder and trial frame with clips. Lens needed are 20D aspheric, 78D aspheric, 90D aspheric, VSFNC super field NC lens, and 4 mirror goinioscopy lens requirement. Ishihara 14 plate book chart, occluder (handle and pinhole) Prism bars: horizontal and vertical Prism Exophthalmometer Auto refractor that also takes auto Keratometer measurements and Lens readings. Portable Slit Lamp Pachymeter with straight probe All items should be easily cleaned to hospital guidelines and joint commission requirements. Vendor will provide training to the Surgery Staff required staff, as well as provide detailed, technical manuals. Grey Market Goods: (a) Gray market items are Original Equipment Manufacturer s (OEM) goods sold through unauthorized channels in direct competition with authorized distributors. This procurement is for new OEM medical supplies, medical equipment and/or services contracts for maintenance of medical equipment (i.e. replacement parts) for VA Medical Centers. No remanufactures or gray market items will be acceptable. (b) Vendor shall be an OEM, authorized dealer, authorized distributor or authorized reseller for the proposed medical supplies, medical equipment and/or services contracts for maintenance of medical equipment (i.e. replacement parts), verified by an authorization letter or other documents from the OEM, such that the OEM s warranty and service are provided and maintained by the OEM. All software licensing, warranty and service associated with the medical supplies, medical equipment and/or services contracts for maintenance of medical equipment shall be in accordance with the OEM terms and conditions. (c) The delivery of gray market items to the VA in the fulfillment of an order/award constitutes a breach of contract. Accordingly, the VA reserves the right enforce any of its contractual remedies. This includes termination of the contract or, solely at the VA s election, allowing the Vendor to replace, at no cost to the Government, any remanufactured or gray market item(s) delivered to a VA medical facility upon discovery of such items.
