2 STATEMENT OF WORK (SOW) Wellness Network Patient Education Jesse Brown VA Hospital 820 S. Damen Ave Chicago, Il 60612 1. PURPOSE The purpose is to procure access to a subscription based patient education television portal or equivalent software for the Jesse Brown VA Medical Center, 820 S. Damen Ave, Chicago, IL 60612. The recommended product is the Wellness Network and the recommended vendor is Interactivation Health Network. This will be a brand name OR EQUAL procurement for a base year and 4 option years. The wellness network will provide customized patient education for all veteran within the inpatient and outpatient setting. The system provides a VA Solutions Library with lists of multiple video content that would be available online as well as the preloaded tablets with the VA Limited Custom level.   Patient education is an important part of care for both the provider and the patient. All too often with time constraints, communication or language barriers, cultural differences, and other issues we face in practice today, patient education may not always end up being the focus of care. Patient education is effective in the transfer of knowledge and skills, but alone is not enough for achieving any type of behavior change. Although by no means a replacement of provider-patient interaction or other educational approaches, videos can be a great way to help supplement the patient education provided during a provider patient encounter. The best method of educational retention comes from approaches such as teach back or motivational interviewing, and when video is used in combination it can significantly improve patient s retention of the education being provided. 2. SCOPE. The successful vendor will license the use of the wellness network software to the Jesse Brown VAMC and support the installation, implementation, and customization of the program. The license grant includes, at no additional charge, the right to make and use a reasonable number of copies of the Software for testing, back-up, archival and disaster recovery purposes, and of the Documentation for internal training, support and deployment. The successful vendor will provide the following solution: Integration of patient education material with health literacy and appropriate patient educational content to meet the needs of the veteran population. Report functionality to enable continual quality improvement and performance tracking; Administrative interface to perform user management, application configuration and auditing; Integrate the wellness network software with the hospital based television software used for evidence-based patient instructions. The wellness network patient education channel will be an add on to our current television system and is customized for the VA environment to help educate veterans about their care and additional VA programs. This may improve the discharge process at a VA site while providing opportunities for clinical staff to be more involved in ensuring that patients do understand their diagnosis and plan of care. This is a new software that will interface with the current cable television to improve patient care and understanding of health diagnosis It allows the care givers to have one visual source in all areas with educational content. Interactivation Health Network grants to the Customer a license for the Customer and its Users to use the Product, during the Term. Term of license is 5 years. Allows care givers to have one visual source ensure patients understand their health diagnosis. Technology that is table integrated application intended to improve the patient experience by providing education regarding patient diagnosis and allows the caregiver to review steps on what to do if a problem arises, assesses the patient s understanding of the plan, gives the patient a written content to take home for review and follow. 3. SPECIFICATIONS. The successful vendor will work with designated Biomedical engineering and IT representatives of the Jesse Brown VAMC to deploy the wellness network. This work will include installation, configuration and customization as required. 3.1 Salient Characteristics/Deliverables: The proposed software will include the following features: Patient education software Creation of health literacy appropriate patient education instructions configured for optimal access by Jesse Brown VAMC staff Reporting functionality to enable continual quality improvement and performance tracking IT and Biomedical Professional interface to perform user management, application configuration and auditing Customized to match Jesse Brown VAMC branding requirements including logo placement Vendor will create and print a Care Plan booklet for every veteran discharged from the Jesse Brown VA Medical Center. 3.2 AHCP Software Installation and Configuration AHCP Software setup. Install the software on the designated server system and create initial user accounts for testing. 3.3 Implementation and Customization of After Hospital Care Plan Vendor will customize the wellness network for the Jesse Brown VAMC concerning specific diagnosis and other areas of healthcare interest to suit the veteran population at Jesse Brown VA Medical Center. 4. PERFORMANCE MONITORING The successful vendor will provide periodic performance reporting during wellness software installation and deployment. Jesse Brown VAMC will have real-time access to the wellness network software during this time and may conduct periodic product sampling to verify compliance with requirements. 5. SECURITY REQUIREMENTS Software shall operate within the VA internal/intranet side of the web system (VAWW). VA sensitive information is not required. The wellness network will work with our current cable network provider to designate a specific channel to stream the educational content. 6. TRAINING Contractor shall provide comprehensive training on the use of the wellness network being purchased for all staff members that are to use this software. At a minimum, training shall include, but not be limited to technical and operational training. Instruction/user guides shall be provided for both training and technical and operational usage. Training shall occur at the Jesse Brown VA Medical Center, 820 S. Damen Ave, Chicago, IL, via on-line, instructor driven classes during normal business hours, 8:00am-4:30pm, Monday through Friday. Training date and time shall be prescheduled and preapproved by the contracting officer s representative prior to initiation. 7. CHANGES TO STATEMENT OF WORK Any changes to this Statement of Work shall be authorized and approved only through written correspondence from the contracting officer or their representative.
