The Department of Veterans Affairs, VISN 08, North Florida/South Georgia VA Medical Center (NF/SG VAMC) is looking for certified, Service Disabled Veteran Owned Small Business (SDVOSB) and Veteran Owned Small Business (VOSB), Small businesses, and/or Large business sources for the following: Home Oxygen Services to include all supplies, materials, equipment, labor, supervision, management, transportation, safety management, infection control, repairs, maintenance and in-service training for patients to include patient education. This is for market research purposes only and is not a request for competitive quotes; however, any business concern that believes it can meet these requirements, please provide written notification to the Contract Specialist within seven (7) days from the date of the publication of this notice. SDVOSB or VOSB must be certified and verified in the U.S. Department of Veteran Affairs, Office of Small & Disadvantaged Business Utilization, Vendor Information Pages web site located at https://www.vip.vetbiz.gov/. Only responses from authorized representatives of the firm will be considered. Adequate knowledge, skills, and abilities providing home oxygen services will be considered. Responses are due by 11/14/2018 4:30 PM, ET. The North America Industry Classification System (NAICS) Code is 532283. Contracting Office Address: Department of Veterans Affairs NCO 08 Acquisitions 300 East University Ave Gainesville FL 32601. Point of Contact(s): Scott Brickey, scott.brickey@va.gov. The contractor will be responsible for providing, installing and servicing medical oxygen equipment, high-pressure cylinder, concentrator and/or liquid systems, portable systems, emergency back-up systems and supplies to eligible VA patients as required by the MRVAMC. The following areas will be serviced under this potential contract: Florida: Counties are covered; Alachua, Baker, Bradford, Calhoun, Columbia, Citrus, Clay, Dixie, Duval, Flagler, Franklin, Gadsden, Gulf, Gilchrist, Hamilton, Jackson, Jefferson, Lafayette, Lake, Leon, Levy, Liberty, Madison, Marion, Nassau, Putnam, St. John s, Sumter, Suwannee, Taylor, Union, & Wakulla. Georgia: Counties are covered; Atkinson, Berrien, Brantley, Brooks, Camden, Charlton, Clinch, Colquitt, Cook, Decatur, Echols, Grady, Lanier, Lowndes, Miller, Mitchell, Pierce, Thomas, and Ware. Performance Monitoring: The contractor's timely performance of requested services will be monitored by the Contracting Officer Representative (COR) and or his or her designees. Services and documentation of care provided under the resultant contract shall be subject to quality management and safety standards as established by VA, consistent with the standards published by TJC or equivalent. The contractor shall develop and maintain Quality Improvement/ Quality Assurance Programs and provision of care equal to or exceeding VA Standards. The results of all Quality Improvement activities performed by the contractor involving VA patients will be shared with VA Quality Management Office. Responsibilities of the contractor: It is the contractor s responsibility to provide all equipment, supplies, and services to support Home Oxygen services. Conflicts of Interest: Contractor has reviewed applicable Federal, State or local conflict of interest laws to assure that conflict of interest situations does not inadvertently arise during this contract.
