Page 1 of 2 THIS IS A SOURCES SOUGHT NOTICE AND IS NOT A REQUEST FOR PROPOSALS, PROPOSAL ABSTRACTS, QUOTATIONS OR INVITATION FOR BIDS   The Department of Veterans Affairs is conducting a MARKET RESEARCH SURVEY to obtain information regarding: (1) the availability and capability of qualified small business sources; (2) whether they are Service-Disabled Veteran-Owned, Veteran-Owned, Small Business, HUB Zone 8(a), Women-Owned, or small disadvantaged business concerns; and (3) their size classification relative to the North American Industry Classification System (NAICS) code for the proposed acquisition. Your responses to the information requested will assist the Government in determining the appropriate acquisition method, including whether a set-aside is possible. An organization that is not considered a small business under the applicable NAICS code should not submit a response to this notice. Small business concerns should be capable of furnishing all labor, supervision, and parts, to include all equipment necessary to perform all work required under the proposed contract. The NAICS code is 621511 DESCRIPTION OF THE REQUIREMENT: Immunohematology Reference Lab Testing Services Reference lab services are required to provide patient testing and product testing for the infusion of compatible or least incompatible blood and blood products in maintaining patient s homeostasis that cannot be initially completed by the VA transfusion service. This reference laboratory will provide the testing, transportation, and product availability necessary for transfusions in a timely manner for urgent and routine transfusions. Due to the complexity of testing and the type of product required (blood and blood components), it is adequate to be performed by a supplier who can perform both the testing and supply the product, even when rare compatibility is evident, and deliver products and testing in a timely manner given the vast complexity of antigens and antibodies in a patient s blood sample that determine patient treatment after transfusion. This confirmatory testing and complex test resolution is provided to establish the safe transfusion of compatible products or least incompatible products to support the transfusion needs of patients. The period of performance is for one year with four optional years of performance. Place of Performance: Charlie Norwood VA Medical Center Downtown Division, 950 15th St, Augusta, GA 30904. POTENTIAL SOURCES SHALL PROVIDE THE FOLLOWING INFORMATION IN THEIR RESPONSE: COMPANY INFORMATION: Company name, address, phone number, primary contact(s), e-mail address, NAICS code(s), business size (i.e., small/large) and DUNS Number. STATEMENT OF CAPABILITY: Submit a brief description, five (5) pages or less, that demonstrates how your company can provide the requested services at time of award. Please include the following: (a) staff expertise including their availability, experience, and formal and other training; (b) current in-house capability and capacity to perform the work; (c) prior completed projects of similar nature; (d) corporate experience and management capability; and (e) examples of prior completed Government contracts, references, and other related information. BUSINESS SIZE AND SOCIO-ECONOMIC STATUS: (a) Indicate whether your business is large or small as defined by the NAICS (b) If small, indicate if your firm qualifies as a small, emerging business, or small disadvantaged business (c) If disadvantaged, specify under which disadvantaged group and if your firm is certified under Section 8(a) of the Small Business Act (d) Indicate if your firm is a certified Hub-zone firm (e) Indicate if your firm is a woman-owned or operated business (f) Indicate if your firm is a certified Service-Disabled Veteran Owned Small Business (SDVOSB) or Veteran Owned Small Business (VOSB) (g) Include the DUNS number of your firm. (h) In accordance with FAR 52.204-7, System for Award Management, prospective contractors must be registered in System for Award Management (SAM) and in VetBiz database if applicable (VetBiz registration is required for Service Disabled Veteran Owned Small Businesses or Veteran Owned Small Businesses) under the applicable NAICS. Contractors may obtain SAM information at https://www.sam.gov and Vet Biz information at http://www.vip.vetbiz.gov. To be eligible for award, contractors must be registered in the System for Award Management database. Registration is free and potential offerors are encouraged to visit the SAM website. DEADLINE: Responses must be emailed to lasonja.harvey@va.gov no later than 11:00 A.M. EDT November 26, 2018. Your response should include both the STATEMENT OF CAPABILITY and BUSINESS SIZE AND SOCIO- ECONOMIC STATUS information as explained above. Please place "Attention: Reference Lab Testing in the subject line of your email. This notice is to assist the VA in determining sources only. A SOLICITATION IS NOT CURRENTLY AVAILABLE. If a solicitation is issued, all interested parties must respond to that solicitation announcement separately from the responses to this announcement.
