Page 1 of 2 VA New England Healthcare System Manchester VA Medical Center 718 Smyth Road Manchester, NH 03104 STATEMENT OF WORK Entomologist Service INTRODUCTION This contract includes but is not limited to: provide all professional consultative services, laboratory testing, tools, supplies, equipment, transportation, and labor to develop and provide written report recommendations for pest control management in building one at the VA Medical Center in Manchester, NH located at 718 Smyth Road. The proposed term of the service is 30 days from the date of the award. DEFINITIONS/ACRONYMS Engineer (ing) - Supervisor or designee. CO: Contracting Officer COR: Contracting Officer's Representative. WHEN (business hours): Weekends, Holidays, Evenings, and Nights. (Anytime outside of Normal Business Hours and Federal Holidays (Monday-Friday 7:00am-4:30pm) ESR: Engineering Service report document containing a summary of the services rendered as outlined under the terms and conditions of the contract. Authorization Signature: COR signature; indicates work status is accepted as stated in ESR. VAMC - Department of Veterans Affairs Medical Center. SCOPE The scope of work (SOW) includes thorough inspection and review of the existing insect problem areas in building one during normal working hours Mon.-Fri. 7am-4:30pm. With After-hours availability at the VA Medical Center, Manchester NH to limit impact to patient care. Work under this contract is to include; but not limited to the following items: Inspect pest management problem areas. Interview area occupants about pest control issues. Review existing medical center pest control and cleaning policies, programs, and procedures. Including services provided by the third-party pest control contractor. Review pest records and other pertinent documentation. Provide initial report of recommendations and findings. Provide formal final written evaluation report of existing conditions and recommendations within 20 days of visit. PERFORMANCE REQUIREMENTS All scheduled service work shall be completed during normal hours (7am-4:30pm). Alternate work hours must be reviewed and approved by the VA Medical Center. All travel cost associated with this contract are to be included in the contract cost. Additional cost will not be given to the Contractor for what they deem out of scope. All labor rates will comply with applicable Wage Rate Determinations by the Department of Labor. COR must be immediately notified of any condition discovered through this contract that could result in extended duration or disrupted service. Consulting services must be provided by a Licensed Pest Control Consultant. Licenses include Associate Certified Entomologist (ACE) or Board Certified Entomologist (BCE). With experience in the Department of Veterans Affairs Healthcare facilities and Medical Centers. Educational requirements include at a minimum a Bachelor s or Master s degree in Entomology.
