Description
The Department of Veterans Affairs, Overton Brooks VA Medical Center, intends to solicit proposal from contractors in the catchment area, Shreveport, Louisiana, to provide services as part of its Contract Residential Care Program for Health Care for Homeless Veterans (HCHV). Safe-restrictive housing will be provided for Veterans to whom care is specifically authorized by the Overton Brooks VA Medical Center located at 510 E. Stoner Ave., Shreveport, Louisiana, 71101. The VA anticipates issuing a solicitation for this requirement on or about December 4, 2018, with a closing date for proposals on or about December 19, 2018. The solicitation will be available for download from www.fbo.gov. Interested contractors will be responsible for downloading copies of solicitation (and any subsequent amendments) from that website, and no hard copies will be made available. The solicitation will provide full information to contractors about the proposal submission process. Prospective offerors are encouraged to register their interest in the solicitation on the FBO website so that they are notified of the issuance of the solicitation and any amendments. Services shall be furnished to beneficiaries for whom such care is specifically authorized by the Department of Veterans Affairs (DVA). Contractor will assist with the provision of emergency residential placement and supportive services through the HCHV program, Safe Haven model. This contract will serve homeless Veterans in need of immediate placement in a safe environment with onsite supportive services. Many of these Veterans have co-morbid conditions that include serious mental illnesses, substance abuse disorders, and other serious medical conditions. The goal of the HCHV Contract Residential Care Program is to rapidly stabilize Veterans medical, mental health, substance abuse and other psychosocial problems in order to place Veterans in other appropriate transitional or permanent housing within 60 to 90 days, but no more than 180 days. VA intends to engage a Contractor to provide rapid placement of VA-referred Veterans in a safe, supportive, residential setting while addressing Veterans complex needs through on-site supportive case management services using a Safe Haven model. This housing must meet the criteria established both by HCHV clinical staff and VA fire and safety officials. It is further understood that beneficiaries receiving services under this contract shall be provided access to basic health and hygiene maintenance opportunity, monitoring of medication compliance, appliances and materials for laundry needs and any other service or supplies without additional charge. The per diem rate established will be an all-inclusive rate. Initial contract award are expected to be on or about March 1, 2019. Contracts will have a base term of one year, with options for four additional one-year terms exercisable at the discretion of the Contracting Officer. The Government will use a best value source selection method under Part 13 of the Federal Acquisition Regulation and anticipates making a firm-fixed price, indefinite delivery contract for this requirement. North American Industry Classification System (NAICS) Code is 623110 and size standard is $27.5 Million and the solicitation will be issued for Full and Open Competition (Unrestricted). All responsible sources may submit proposals which shall be considered by the Agency. To be eligible for award, contractors must be registered in SAM (System for Award Management) database (information on registration requirements can be obtained by the Internet at https://www.sam.gov and submit their complete Annual Representations and Certifications (ORCA) via online at https://orca.bpn.gov at proposal due date/time.
