This announcement constitutes the only solicitation; quotations are being requested, and a written solicitation document will not be issued. This solicitation is a Request for Quotations (RFQ) and the solicitation number is 36C24418Q0444. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-100 effective August 22, 2018. The associated North American Industrial Classification System (NAICS) code for this procurement is 562991, Septic Tank and Related Services. The size standard for business concerns competing is unrestricted. The Department of Veterans Affairs, Lebanon VA Medical Center, 1700 South Lincoln Avenue, Lebanon, PA 17042-7529 is seeking to obtain Portable Restroom Trailer Service. All interested companies shall provide quotations for the following: The anticipated Period of Performance will be 8 Months (12-03-2018 to 07-31-2019) following Contract award and includes the following Contract Line Item Numbers (CLIN): CLIN 0001: Trailer Rental & Servicing for the YMCA Portable Restroom usage in accordance with the Statement of Work (12-03-2018 to 07-31-2019). CLIN 0002: Equipment Mobilization/Demobilization in accordance with the Statement of Work. (12-03-2018 to 08-02-2019). See also Attachment 2, Price Cost Schedule. The SOW is included as Attachment 1 to this solicitation. Offerors are requested to quote Firm Fixed Price for any and all of the line items on the Schedule of Supplies and Services attached to this solicitation. The Schedule of Supplies and Services is included as Attachment 2 to this solicitation. Service Contract Act Wage Determination Number 2015-4231, Revision Number 6, dated 07-11-2018, applies to this acquisition and will be incorporated into the resulting contract. A copy of the Wage Determination is included with this solicitation as Attachment 3. Map of YMCA location at LVAMC is included as Attachment A. Map of area of required restroom location is included as Attachment B. Any award resulting from this solicitation will be issued on a Standard Form 1449.
