In accordance with FAR 15.201(e), responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. Responders are solely responsible for all expenses associated with responding to this sources sought. Small Business, Service Disabled Veteran Owned, Veteran owned, Women Owned, etc Email Address Phone Number Physical Address Capability statement pertaining to Scope of Work Introduction The Veterans Affairs Pittsburgh Healthcare System (VAPHS) is planning to expand its music therapy program. To support this plan, VAPHS is seeking a contractor who shall provide two (2) musicians: one skilled with a harp and one skilled as a Drum Circle Facilitator. Both musicians shall possess the appropriate credentials as described in this Statement of Work. The contract shall be firm fixed BPA for 5 years. Locations Services shall be provided at both the H.J. Heinz and University Drive campuses: VA Pittsburgh Healthcare System H.J. Heinz Division 1010 Delafield Road Pittsburgh, PA 15215 University Drive (Oakland) Division University Drive C Pittsburgh, PA 15240 The services will be provided either bedside or in a small group, two (2) to five (5) veterans in a unit day room. The Contractor shall coordinate all visits with the VA contract officer representative (COR) at a mutually agreeable schedule. Harp The Harpist shall possess current certification as a Certified Music Practitioner (CMP) from the Music for Healing and Transition Program (MHTP.org) or equivalent A CMP is necessary to provide enhanced music programming specifically targeted towards palliative and hospice veterans with bedside music through Harp Intervention. A CMT is a high level professional musician with a certification as a health practitioner; knowledgeable in health, wellness, disease processing; proficient in observations of physiological cues and communication; skilled in music delivery for bedside or intimate music delivery to bring about positive outcomes to improve client welfare physically, emotionally, socially and spiritually. Additionally, research has indicated the presence of live music is much more beneficial than prerecorded as delivery is specific to the client, through social engagement. The Harp CMP shall possess a large repertoire to enable them to instantaneously response to the needs, desires and physiological/psychological changes present in the intimate setting bedside. The Harp CMP must have at least 5 years of experience working. The CMP must provide certificate from MHTP and a resume with evidence of the 5 years experience that includes specific agencies where contracted or employed, contact persons and dates of service. Harp services shall be provided once per week for a minimum of 90 minutes each week. A typical weekly visit will consist of 10 to 15 minute sessions in 5 to 10 different rooms. All instruments shall be provided by the contractor. The small Irish harp, is portable and able to fit in the smallest of unit rooms on medical floors. The lightweight carbon fiber harp is easily moved from room to room and can be cleaned after leaving one room for the next. A list of available instruments shall be provided with the quote. Drums The Drum Circle Facilitator shall possess current certification as must be board certified music therapist (MT-BC) from The Certification Board for Music Therapist (CBMT) or equivalent. The Drum Circle Facilitator shall possess the skills to lead groups in drumming with those who have severe mental and physical disabilities and disorders. The facilitator must have at least five years as a MT-BC and five years as a drum circle facilitator. They must provide certification from Certification Board of Music Therapy and a resume with evidence of the 5 years experience that includes specific agencies where contracted or employed, contact persons and periods of service. Drum circles sessions are tentatively scheduled outdoors once per month in June, July, August, and either May or September and indoors in January and in a month TBD for a total of six (6) visits per contract year. The Drum Circle Facilitator services shall be provided for a minimum of one (1) hour drum each visits. All instruments shall be provided by the contractor. The Drum Circle Facilitator shall provide the a minimum quantity of ten (10) hand drums (e.g. conga, djembe, tubano) and a minimum quantity of ten (10) hand held percussion (e.g. tambourine, ocean drum, bell, shakers) for up to 15 veterans to utilize during each event. These must be clean with no sharp edges. As contact by veteran is involved, the contractor shall clean all instruments before and after use. A list of available instruments shall be provided with the quote. Security Requirements Contractor personnel are required to follow all Security Requirements that are listed within the contract. The VA Privacy and Information Security Awareness and Rules of Behavior certificate will need to be emailed to the Contracting officer representative for all personnel coming on-site. Influenza VHA Directive 1192 requires all health care personnel (HCP) to participate in the seasonal influenza prevention program and outlines the key implementation steps. Health care personnel is defined in VHA Directive 1192 as all VA licensed and unlicensed, clinical and administrative, paid and unpaid, full- and part-time employees, intermittent employees, fee basis employees, VA contractors, students, researchers, volunteers, and trainees who work in VHA locations. HCP covered by the policy are expected to receive annual influenza vaccination. Those HCP unable or unwilling to be vaccinated are required to wear a face mask throughout the influenza season. Masks are available at entrances throughout the medical center. It is the responsibility of the contractor to ensure that all contract staff is compliant with the requirements outlined in VHA Directive 1192. The contractor shall maintain the following documentation: - Documentation of vaccination, e.g., signed record of immunization from a health care provider or pharmacy, or a copy of medical records documenting the vaccination. - Completed Health Care Personnel Influenza Vaccination Form (Attachment B of VHA Directive 1192) if unwilling or unable to receive the vaccine. The contractor is required to submit annual certification to the Contracting Officer s Representative (COR) that all contract staff performing services at VA facilities are in compliance with VHA Directive 1192. Tuberculosis Contractor is required to annual certify all Contractors working on VAPHS have documented screening for latent Tuberculosis within the last 12 months and every 12 months thereafter while working at VAPHS. If screening for latent Tuberculosis is positive, clearance from a physician is required prior to working at VAPHS. The Contractor shall also be notified that anyone failing to meet these requirements is not permitted to work onsite and/or may be removed at any time without notice until these requirements are met. Provide evidence to respective COR. General Requirements Contractor will schedule with the COR for all services. Upon arrival at VAPHS, the contractor and employees shall sign in with the VA Police Department and obtain a vendor/contractor access badge. The VA badge shall be plainly visible by VA staff and must be worn above the waist, facing forward. Upon leaving the facility, the contractor and employees shall return the access badge to the VA police and sign out of the facility. All travel expenses are the responsibility of the contractor. Press releases, marketing material, or any other printed or electronic documentation related to this project, shall not be publicized without the written approval of the CO.
