- This is a SOURCES SOUGHT ANNOUNCEMENT ONLY. It is neither a solicitation announcement nor a request for proposals or quotes and does not obligate the Government to award a contract. Requests for a solicitation will not receive a response. Responses to this sources sought must emailed to Joseph.Cellino@va.gov prior to the posted response date and time. The purpose of this sources sought announcement is for market research to make appropriate acquisition decisions and to gain knowledge of potential qualified Service Disabled Veteran Owned Small Businesses, Veteran Owned Small Businesses, 8(a), HubZone, other Small and Large Businesses interested and capable of providing as needed hotel lodging as described in the draft statement of work below. Documentation of capability must be described in sufficient detail for the Government to determine that your company possesses the necessary functional area expertise, experience and/or facilities to compete for this acquisition. Responses to this notice shall include the following: (a) company name (b) address (c) point of contact (d) phone, fax, and email of point of contact (e) DUNS number (f) provide a capability statement that addresses the organizations qualifications and ability to provide the services listed below. Anticipated Period of Performance: April 01, 2019 through March 31, 2020, with four subsequent option years. DRAFT STATEMENT OF WORK REQUIREMENTS: Indigent Funeral Services and transportation of remains for Buffalo and Batavia VA Medical Center(s) located at: Buffalo VAMC 3495 Bailey Avenue Buffalo, NY 14215 Batavia VAMC 222 Richmond Avenue Batavia, NY 14020 The Bath National Cemetery is located at San Juan Avenue, Bath, NY 14810. Period of Performance: 04-01-2019 through 03-31-2020, with four subsequent option years Requirements: The contractor shall place the body in a casket in such a manner as to create an appearance of rest and composure and to assure maintenance of position during shipment or local burial. The Contractor shall be responsible for providing professional services of the highest quality to assure viewing of the remains under optimal conditions. The Contractor shall practice hygienic measures that will assure complete and satisfactory disinfection and sanitation of the funeral establishment. The Contractor shall have a sufficient number of licensed embalmers to process (embalm) or reprocess any remains on a timely basis. Interns or apprentices may be used to assist the licensed embalmer in accordance with (IAW) with applicable state regulations. Transport Vehicles, supplies, equipment and technical procedures shall conform to standards and professional techniques acceptable to the New York State Funeral Service Industry and the National Cemetery indicated above. The Contractor facilities shall be within a 10-mile radius from the Buffalo VA Medical Center. Contractor s Facility shall meet Federal, State, Local and National Environment Policy Act (NEPA) Fire and Life Safety Codes. The Contractor s facility shall comply with current VA standards for handicapped accessibility. Any change of the Contractor s facility location during the contract term shall be approved in advance by the Contracting Officer (CO). Casket: Material: That the casket shall be, at a minimum, constructed from thick, strong particle board and be of sufficient strength to support the weight of an adult human body. Cardboard or pressed paper of similar materials are not acceptable. Sizes: Caskets shall be furnished in stock sizes and shall be of sufficient length and width to contain the body without crowding. Extra-sized caskets shall be furnished at no additional cost. Flag: Contractor will ensure that a U.S. Flag (furnished by the VA) accompany and drape the casket at the time of the funeral service. Clothing: Unless it is determined by the VA, (and indicated at the time the funeral arrangements are made) the contractor shall dress the body in prescribed undergarments and outer clothing furnished by contractor or by relatives or friends of the deceased. Transportation of the Body: When full services are provided the Contractor will provide transportation of the body from the Medical Center to the place of preparation in a hearse or service car with subsequent transportation to the location where final burial and services will be held. Should the interment or cremation require transportation to a location other than the Federal Cemetery in Bath, NY, additional expenses may be allowed for transportation as determined by Contracting Officer s Technical Representative (COR). When transportation but full services are not required the Contractor will be responsible for transportation of the body for storage or autopsy according to the needs of VA; from one Medical Center to the other or from one medical center to Erie County Medical Center. Cremation of Body: In the event cremation services are ordered by the VA, this service shall include making all necessary arrangements with the crematorium and furnishing a suitable container for the cremated remains. Inspection: The casket and remains will be inspected by a representative of the VA prior to remains leaving the contract funeral home.
