- SOURCES SOUGHT DESCRIPTION This is NOT a solicitation announcement. This is a sources sought/RFI only. The purpose of this sources sought/RFI is to gain knowledge of potential qualified sources and their size classification relative to NAICS 339115 (size standard of 1000 Employees). Responses to this sources sought will be used by the Government to make appropriate acquisition decisions. After review of the responses to this sources sought, a solicitation announcement may be published. Responses to this sources sought synopsis are not considered adequate responses for a solicitation announcement. The Department of Veterans Affairs (VA), VISN 21 Network Contracting Office, is seeking Service Disabled Veteran Owned Small Businesses (SDVOSB), and / or Veteran Owned Small Businesses (VOSB) sources that can provide Haag Streit Tonometer Measuring Prisms, Volk Hand Held Gonio Lenses, and Volk Gonio Fundus Laser Lenses that at a minimum meets the following salient characteristics with a brand name or equal product for the VA San Jose Outpatient Clinic: Haag Streit Reusable Tonometer Prisms 1) Prisms must be compatible for use with the Haag Streit AT900 Applanation Tonometer. 2) Prisms are reusable and therefore need to be durable for repeated cleaning and use. 3) Prisms must be able to stand disinfection by a number of commercially available disinfectants (except alcohol). 4) Prisms can be used with both fixed or hand held tonometer s. 5) They are required to be mechanically precise and reliable in their measurements. Volk G 3 Three Mirror Glass Gonio Fundus Lens Mirrors are accurately angled to eliminate gaps in the visualized fundus 1) All glass design provides superior clarity and durability compared to acrylic lenses 2) High quality version of the commonly named Goldmann lens 3) Available in numerous contact options including a mini version for ease of use 4) Lens case included 5) Field of view: 60 °/66 °/76 ° 6) Image Magnification: 1.06x 7) Laser spot:.94x 8) Working distance: 15mm Volk G 4 Four Mirror Glass Gonio Lens, No Flange, No Fluid 1) Four precisely angled mirrors provide full 360 ° views of the anterior chamber. 2) All glass design provides superior clarity and durability compared to acrylic lenses. 3) Available in numerous contact options for ease of use. 4) Large ring (29.5mm) and no flange no fluid. 5) Lens case included. 6) Field of view: 4x64 ° 7) Image Magnification: 1.0x 8) Laser spot: 1.0x 9) Working distance: 8.4mm The information identified above is intended to be descriptive, not restrictive and to indicate the quality of the supplies/services that will be satisfactory. It is the responsibility of the interested source to demonstrate to the government that the interested parties can provide the supplies/services that fulfill the required specifications. If you are interested, and can provide the sought out supplies/services, please provide the requested information as well as the information indicated below. Response to this notice should include company name, address, point of contact, size of business pursuant to the following questions: (1) Please indicate the size status and representations of your business, such as but not limited to: Service Disabled Veteran Owned Small Business (SDVOSB), Veteran Owned Small Business (VOSB). (2) Is your company considered small under the NAICS code identified under this Sources Sought/RFI? (3) Are you the manufacturer or distributor of the items being referenced above (or equivalent product/solution)? (4) If you re a small business and you are an authorized distributor/reseller for the items identified above (or equivalent product/solution), do you alter; assemble; modify; the items requested in any way? If you do, state how and what is altered; assembled; modified? (5) Does your company have an FSS contract with GSA or the NAC or are you a contract holder? If so, please provide the contract type and number. (6) If you are an FSS GSA/NAC or NASA SEWP contract holder or another federal contract holder, are the referenced items/solutions available on your schedule/contract? (7) Please provide general pricing for your products/solutions for market research purposes. (8) Please submit your capabilities regarding the salient characteristics being provided and any information pertaining to equal to items to establish capabilities for planning purposes? *** Submissions addressing Section (8) should show clear, compelling and convincing*** evidence that all equal to items" meet all required salient characteristics. In accordance with the U.S. Supreme Court decision regarding Kingdomware, service disabled veteran owned businesses (SDVOSB) and veteran owned businesses (VOSB), who are interested in this procurement and consider themselves to have the resources and capabilities necessary to provide these services shall be verified in VetBiz Registry as follows to be considered: http://www.vip.vetbiz.gov. Process in 38 U.S.C. 8127(d) whereby a contracting officer of the Department shall award contracts based on competition restricted to small business concerns owned and controlled by veterans, if the contracting officer has a reasonable expectation that two or more small business concerns owned and controlled by veterans will submit offers, and that the award can be made at a fair and reasonable price that offers the best value to the United States. For purposes of this VA specific rule, a service-disabled veteran-owned small business (SDVOSB) or a veteran-owned small business (VOSB), must meet the eligibility requirements in 38 U.S.C. 8127(e), (f) and VAAR subpart 819.7003 and be listed as verified in the Vendor Information Pages (VIP) database. Last, as per VAAR 810.001-70 Market Research Policy, all SDVOSBs and VOSB shall be verified under the NAICS code noted in the requirement to be considered. Any SDVOSB or VOSB not verified in Vet.biz under the NAICS code utilized by the Contracting Officer will not be considered for the requirement. (see below VAAR reference). Responses must be received no later than Thursday, December 13, 2018 at 09:00 a.m. PST. If a solicitation is issued it shall be announced later, and all interested parties must respond to that solicitation announcement separately from the responses to this request for information. Responses to this notice are not a request to be added to a prospective bidders list or to receive a copy of the solicitation.
