- Source Sought Notice for Medical Physics Certification Services for Radiology at Columbus VAACC, Dayton VAMC, Cincinnati VAMC and Chillicothe VAMC The purpose of this sources sought notice is to seek responses from all qualified Small Business prospective offerors who hold a current American Board of Radiology (ABR) or equivalent certification to perform medical physics certification services in the state of Ohio to determine the set-aide type (SDVOSB, VOSB or Small Business) for the forthcoming solicitation summarized as follows: Short Title: Medical Physics Certification Services for Radiology NAICS-541690: Professional, Scientific, and Technical Services PSC-Q522: Medical Radiology Purpose: The purpose of this sources sought notice is to determine what type of set-aside is appropriate for this acquisition (SDVOSB, VOSB or Small Business). Places of Performance: Chalmers P. Wylie VA Ambulatory Care Center (VAACC), 420 North James Road, Columbus, Ohio 43219 Dayton VA Medical Center (VAMC), 4100 W. Third Street 45428, Dayton, Ohio Cincinnati VA Medical Center (VAMC), 3200 Vine Street 45220, Cincinnati, Ohio Chillicothe VA Medical Center (VAMC), 17273 State Route 104, 45601, Chillicothe, Ohio Anticipated Period of Performance: 25 February 2019 to 24 February 2024 comprised of a base year and four option years. General Requirements: The contractor shall provide all vehicles, personnel, management, transportation, materials, supplies, and equipment necessary to perform contract services as defined and specified in the PWS which shall be provided in the forthcoming solicitation. The contractor shall perform services under this contract in strict adherence to all Federal, State and local laws and regulations unless supplemented to exceed those requirements as specified in the PWS. Anticipated Requirements: The anticipated not-to-exceed (NTE) quantities for this requirement are provided in the attached SCHEDULE OF SERVICES PRICE SCHEDULE. Attachments for Reference: Draft SCHEDULE OF SERVICES PRICE SCHEDULE is attached for reference as appropriate. Draft Performance Work Statement (PWS) is attached for reference as appropriate. Request all Small Business prospective offerors interested in this requirement email their company name, DUNS number, CAGE code, small business socio-economic size status and a signed copy of their current ABR certification in Adobe PDF format to perform medical physics certification services for the subject anticipated requirement to bradley.burhite@va.gov not later than 9 January 2019. Only those prospective small business prospective offerors who are certified under NAICS-541690 (or equivalent) and submit a current signed copy of their ABR certification in response to this sources sought notice will be considered responsive for the purposes of this FBO posting.
