Description
NOTICE OF INTENT TO SOLE SOURCE The Department of Veterans Affairs intends to issue sole-source firm-fixed priced award contract, to Canon Medical Systems USA, Inc., 2441 Michelle Dr, Tustin, CA 92780-7047. A sole source procurement will be issued to Canon Medical Systems USA, Inc. because they are the only vendor capable of fully performing the service. Due to the proprietary & specialized nature of the work it must be conducted by OEM personnel with a high level of training & qualifications. Canon Medical Systems USA, Inc. is the owner of the proprietary software and does not offer or authorize the software to a third party vendor. Imaging systems are only to be repaired by Canon technicians. This procurement is being conducted in accordance with FAR Part 6.302-1 Only One Responsible Source and No Other Supplies or Services Will Satisfy Agency Requirements. The NAICS code is 811219 and has a size standard of $20.5 Million for this procurement. Statement of Work 1.0 GENERAL Contractor shall provide personnel to complete the task of providing full service to Aquilion CT Scanner & two Aplio 500 Ultrasound machines located at the Department of Veterans Affairs Chalmers P. Wylie Ambulatory Care Center (VA ACC), Columbus, Ohio. 1.1 CONTRACTING OFFICER S REPRESENTATIVE (COR) The Contracting Officer s Representative (COR) will be the principal liaison for the Contractor during the duration of the contract and must be contacted prior any services performed on the equipment. 2.0 SPECIFIC TASKS Provide both planned maintenance and emergency service according with VA NAC Contract SPE2D1-17-D-0022. Provide both planned maintenance and emergency service for Aquilion CT Scanner & two Aplio 500 Ultrasound and associated Transducers. Provide all repairs and calibrations to these instruments and associated components. Provide all parts and labor (and return shipping costs if needed) associated with the repair and/or calibration of these instruments as incurred during this contract. Check in Requirements: The Field Service Engineer must report to the Biomedical Engineering Department located in 1B090 of the Ambulatory Care Center to sign in with Biomedical Engineering before work begins. Submit any mobile media devices that would be used on the system to a virus scan. Upon completion of work, the Field Service Engineer must report to Biomedical Engineering to sign out and brief Biomedical Staff or Supervisor if Biomedical Staff is unavailable concerning completion of service. At the end of briefing Field Service Engineer will sign out in the Biomedical Engineering Department. Biomedical Engineering can be reached at 614-388-7273. Documentation/Reports: The Contractor shall submit a legible field service report, which shall include detailed descriptions of the inspection and/or emergency repair services performed, including replaced parts and estimated prices required for the service call within 15 days of service being provided. NOTE: Any additional charges to be claimed must have been approved by the COR before service was performed. Test Equipment: Upon request of the COR or the Contracting Officer, the Contractor shall provide a copy of the current Calibration Certification of all test equipment which is to be used by the Contractor to perform service under this contract. Calibration of equipment shall be traceable and in conformance with test equipment Original Equipment Manufacturer standards. SUPPORTING INFORMATION Service Employee Qualifications: Service Employees shall be competent to perform the specific repairs. Hours of Work: Contract employees will work during hours 8:00am 9:00pm, M-F excluding Federal Holidays. Location of Work: All work will be performed at the VA ACC, 420 N. James Road; Columbus, Ohio 43219. This notice of intent is not a request for competitive quotes. However, interested parties may identify their interest and capability to respond to the requirement and submit quotations no later than 4pm EST, Dec. 26, 2018 to: Michael.groneman@va.gov. Responses received will be evaluated; however, a determination by the Government not to compete the proposed procurement based upon responses received to this notice is solely within the discretion of the Government. Determination by the Government not to compete the proposed procurement based upon responses received to this notice is solely within the discretion of the Government.
