This announcement constitutes the only solicitation; Quotes are being requested, and a written solicitation document will not be issued. This solicitation is a request for quotations (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular Number 2005-101; effective October 26, 2018. The associated North American Industrial Classification System (NAICS) code for this procurement is 339112 (Surgical and Medical Instrument Manufacturing) with a small business size standard of 1000 employees. The Department of Veterans Affairs, William S. Middleton VA Medical Center, 2500 Overlook Terrace located in Madison, WI 53705-2286 is seeking to purchase Brand Name ManoScan Catheters. These Catheters are needed to replace the existing Catheters that are over 4 years old and end of life. The Madison VA Medical Center currently has a Medtronic Esophageal Manometry system in use and the current catheters are over 4 years old. This machine is used to provide useful information to aid in the diagnosis of conditions such as dysphasia, achalasia, and hiatal hernias. Delivery and Complete Original Equipment Manufacturer Warranty is required to allow the high standard of patient monitoring and clinical diagnostics to continue for Our Nation s Veterans within the Madison VA Medical Center located at Madison, WI 53705-2286. Salient Characteristics required to meet the Government s needs: 1. Catheters must be compatible with the Manoscan equipment already present in the Ambulatory Surgery/6A department. 2. Catheters must have 36 channels provide 432 points of measurements to create a pressure image from pharynx to stomach. 3. All sensors must be true circumferential. 18 impedance channels display bolus transition from pharynx to stomach. 4. 96 3D channels provide a 3- dimensional EGJ visualization. All interested SDVOSB s who are authorized distributors for the Original Equipment Manufacturer shall provide Quotes for the following BRAND NAME items: Supplies Line Item Description Quantity Unit Price Total Price 0001 ITEM NUMBER: 3890. ManoScan ESO Z Catheter. The ManoScan ESO high resolution manometry catheters allow medical professionals to obtain complete physiological map from the pharynx to the stomach. The catheters create a pressure image using 36 channels and 432 points of measurement. The ManoScan ESO high resolution manmetry system enables full evaluation of the motor functions of the esophagus. 2 EACH 0002 ITEM NUMBER: 80372, HRM Catheter Extended Warranty, 1 Year 2 EACH GRAND TOTAL $ Services 1. All line items (CLINs) for the ManoScan ESO Z Catheters and HRM Catheter Extended Warranty will be delivered to the Department of Veterans Affairs, William S. Middleton VA Medical Center, 2500 Overlook Terrace, Madison, WI 53705-2286. All line items listed above are Brand Name. There is an attaching Brand Name Justification accompanying this action. The delivery/task order period of performance is as soon as possible after contract award has occurred. Delivery shall be provided no later than 30 days after receipt of order/award of contract. Award shall be made to the offeror whose quotation offers the lowest price technically acceptable. 2. Price reasonableness, in accordance with FAR 13.106-3, will be followed. 3. For authorized distributors of the Brand-Name or Equal, proof of authorized distributorship must be included in the submission to the Contracting Officer. The Contracting Officer will deem the submission an un-interested party if authorized distributorship paperwork from the Original Equipment Manufacturer is not received or missing with submission. 4. This is an open-market combined synopsis/solicitation for products and services, as defined herein. The government intends to award a contract as a result of this combined synopsis/solicitation that will include the terms and conditions set forth herein. In accordance with FAR 12.301(b)(1) and FAR 52.212-1(b)(11), To facilitate the award process, all quotes must include a statement regarding the terms and conditions herein as follows: "The terms and conditions in the solicitation are acceptable to be included in the award document without modification, deletion, or addition." OR "The terms and conditions in the solicitation are acceptable to be included in the award document with the exception, deletion, or addition of the following:" Offeror shall list exception(s) and rationale for the exception(s). 5. The Offerors quote submitted to this CO shall also have the same 2-line item that matches this notice, and the required statement regarding terms and conditions of contract within their quote. The Offeror may submit this same Combined Synopsis/Solicitation as their official quote, and if so, all pages are required for submission, and the vendors name and DUNS number must be placed on the last submitted sheet of this Combined Synopsis/Solicitation. The following solicitation provisions apply to this acquisition: FAR 52.212-1, Instructions to Offerors Commercial Items JAN 2017 (a) Gray market items are Original Equipment Manufacturers (OEM) goods sold through unauthorized channels in direct competition with authorized distributors. This procurement is for new OEM medical supplies, medical equipment and/or services contracts for maintenance of medical equipment (i.e. replacement parts) for VA Medical Centers. No remanufactures or gray market items will be acceptable. (b) Vendor shall be an OEM, authorized dealer, authorized distributor or authorized reseller for the proposed medical supplies, medical equipment and/or services contracts for maintenance of medical equipment (i.e. replacement parts), verified by an authorization letter or other documents from the OEM, such that the OEM s warranty and service are provided and maintained by the OEM. All software licensing, warranty and service associated with the medical supplies, medical equipment and/or services contracts for maintenance of medical equipment shall be in accordance with the OEM terms and conditions. (c) The delivery of gray market items to the VA in the fulfillment of an order/award constitutes a breach of contract. Accordingly, the VA reserves the right to enforce any of its contractual remedies. This includes termination of the contract or, solely at the VA s election, allowing the Vendor to replace, at no cost to the Government, any remanufactured or gray market item(s) delivered to a VA medical facility upon discovery of such items.
