The VA NJ Healthcare System is seeking services for quarterly testing of dental water for Bioburden Colony count for the East Orange, Lyons and Brick Medical Campuses. (See... Place of Performance in Section B of SOW) All interested companies shall provide quotation(s) for the requested service, which includes sampling kits, delivery to each location, receiving samples, testing, and reporting test results electronically via email to the designee, Contracting Officer s Representative (COR). Howard Outpatient Clinic - 970 NJ-70, Brick, NJ 08724 52.212-2 Evaluation-Commercial Items (OCT 2014) Award shall be made to the offeror whose quote offers the best value to the government IAW FAR Subpart 13.106-1(a)(2). The government will evaluate information based on the following evaluation criteria: (1) technical capability factor "meeting or exceeding the requirement, (2) past performance, and (3) price (is an important factor).     Technical Capability Factor: The Offeror shall provide a maximum 10-page capability statement that indicates the vendor has the trained personnel, equipment, and track record of working experience in providing sampling kits and testing of Bioburden Colony Count for dental facilities. The 10-page limit only applies to this section, information requested below are not included in the page count. The Offeror shall include documentation on the company s quality control, storage, and testing process. The Offeror shall indicate how shipping to and from the VA locations shall be addressed. The Offeror shall provide certifications for all Certified Laboratory Technicians that are assigned laboratory work from New Jersey VA Healthcare System before work commences. If uncertified technicians are being used the vendor shall provide proof of a formal in-house training program run by a certified technician. Any subcontractors used in the performance of this requirement shall be identified and the required certifications and training program information shall be included with the package. Past Performance: The Offeror shall provide a list of contracts completed during the past three (3) years, all contracts and subcontracts currently in process for dental laboratory services. Include the following information for each contract: Name and address of contracting activity Name and telephone number of contract person Dates of contract performance The contractor will be evaluated based on the following: reliability, cost, order accuracy, delivery/timeliness, quality, business relations, personnel, customer support, and responsiveness. Relevant past performance information shall include key personnel who have relevant experience, predecessor companies, and subcontractors who will perform major or critical elements of this solicitation. The contracting office will also review performance history in CPARS. Offerors without relevant past performance or for whom past performance information is not available, will receive a neutral rating. Failure to provide the information requested in the evaluation criteria may result in package being found incomplete. Price: Vendor s price must be fair and reasonable as determined by Contracting Officer s price analysis. All interested companies shall provide quotation(s) for the requested service, which includes sampling kits, delivery to each location, receiving samples, testing each sample, and reporting test results electronically via email to the designee, Contracting Officer s Representative (COR). Options: The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s). A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. This is an open-market combined synopsis/solicitation for testing of dental water for Bioburden Colony Count as defined herein.    The government intends to award a contract as a result of this combined synopsis/solicitation that will include the terms and conditions set forth herein for a Veteran-Owned Small Business total set-aside. To facilitate the award process, all quotes must include a statement regarding the terms and conditions herein as follows: "The terms and conditions in the solicitation are acceptable to be included in the award document without modification, deletion, or addition."
