- This is a Sources Sought Announcement Only. It is neither a solicitation announcement nor a request for proposals or quotes and does not obligate the Government to award a contract. Requests for a solicitation will not receive a response. Responses to this Sources Sought must be in writing. The purpose of this Sources Sought announcement is for market research to make appropriate acquisition decisions and to gain knowledge of potential qualified Service-Disabled Veteran Owned Small Businesses, Veteran Owned Small Businesses, and other Small Businesses interested and capable of providing the services described below. Documentation of technical expertise must be presented in sufficient detail for the Government to determine that your company possesses the necessary functional area expertise and experience to compete for this acquisition. Responses to this notice shall include the following: (a) company name (b) address (c) point of contact (d) phone, and email (e) DUNS number (f) Cage Code (g) Tax ID Number (h) Type of small business (e.g. Services Disabled Veteran Owned Small Business, Veteran-owned small business, etc.). Vendor must provide a capability statement that addresses the company s qualifications and ability to perform as a contractor for the work described below. The VA Pacific Islands Health Care System (VAPIHCS) located at Honolulu, HI is seeking a qualified contractor that can provide preventative maintenance (PM) and repair on one Hologic Horizon A DXA equipment in accordance with the Hologic Manufacturer specifications. All professionals providing services under the terms of this contract must be appropriately certified and possess the skills and experience to perform the services as required. The NAICS code for this acquisition is 811219 ($22M). All offerors must be registered in the System for Award Management (SAM) at www.sam.gov. All offerors should submit their information by email to Jocelyn.layson-castillo@va.gov. Submissions should be received no later than Monday, December 2nd by 3:00pm HST. (See draft Statement of Work). STATEMENT OF WORK FOR SERVICE MAINTENANCE AND REPAIR OF HOLOGIC DEXA SCANNER FOR RADIOLOGY SERVICE, VAPIHCS HAWAII Statement of Work: The vendor shall provide all hardware, software and labor for the maintenance and repair of the Hologic DEXA scanner at VAPIHCS Radiology. Period of Performance: From: February 1, 2020 To: March 31, 2021 Option Year 1: February 1, 2021 To: March 31, 2022 Option Year 2: February 1, 2022 To: March 31, 2023 Option Year 3: February 1, 2023 To: March 31, 2024 Option Year 4: February 1, 2024 To: March 31, 2025 Location Information: The VA Pacific Islands HCS Radiology Service is located on the 2nd floor of 459 Patterson Road, Honolulu Hawaii 96819-1522. Specifications: Vendor Shall Provide Under This Agreement: Preventative Maintenance Labor for one (1) scheduled preventative maintenance inspections per twelve-month period per unit, to occur between the hours of 8:00 a.m. and 4:30 p.m., Monday through Friday, holidays and weekends excluded. Vendor will perform all required services to the Equipment in accordance with the maintenance program, which is set forth in applicable service manuals. Contractor shall provide complete inspections, testing, annual preventive maintenance, repairs for the Hologic Horizon A DEXA scanner to include, Hologic product hardware and software upgrades; full parts and service coverage for term of agreement. Contractor shall furnish tools, test equipment, test instruments, cleaning materials, and parts required to perform all services. All work shall be performed Monday through Friday between 8:00am and 5:00pm, exclusive of Federal Holidays. Telephone support will be provided between 8 a.m. - 5 p.m., Monday - Friday, holidays and weekends excluded. Labor and Travel for visits to repair the Equipment will be provided, Monday through Friday from 8:00 a.m. - 5:00 p.m., as necessary. For each visit, Contractor must notify, coordinate, and schedule all services (routine or unscheduled) with Radiology Department. Contractor will provide Radiology Service a copy of the service report. Results shall contain the following data: 7a. Date of service 7b. Work order #, service call #, etc. 7c. Equipment data: type, model, serial number 7d. Results and summary of services performed 7e. Contractor/Technician info: printed name, identification number (if applicable) Invoices will be paid upon final approval of the contracting Officer Representative (COR), in which completion of services is verified, and that all requirements in SOW is met. Parts may include, at vendor's discretion, new, used, refurbished or equivalent parts. Replaced parts become the property of vendor. All parts required for maintaining the Equipment shall be included. Electronic storage media and IT equipment that stores or processes VA information by, or on behalf, of the Department of Veterans Affairs (VA) (e.g. hard drives) will not leave VAPIHCS facility until Biomedical Engineering Service has completed procedures for sanitization that mitigate the risks of unauthorized disclosure of patient information and to ensure its confidentiality.
