Description
This is a SOURCES SOUGHT NOTICE for market research purposes only to determine the availability of potential businesses with capabilities to provide the services described in the Statement of Work (SOW). Potential offerors are invited to provide information via e-mail to susan.delacruz@va.gov. All responses will be used to determine the appropriate acquisition strategy for a potential future acquisition according to SOW. Potential contractors shall provide, at a minimum, the following information to susan.delacruz@va.gov: 1) Name, address, point of contact name, phone number, and e-mail address, duns number, and socioeconomic category. 2) Brief capability statement with enough information to determine if the company can meet the requirement. The Capabilities Statement for this sources sought is not expected to be a Request for Quote nor does restrict the Government to an ultimate acquisition approach, but rather short statement regarding the company's ability to provide the service below. Any commercial brochures or currently existing marketing material may also be submitted with the capabilities statement. The capabilities will be evaluated solely for the purpose of determining to Set-Aside for the Small Business Community or to conduct as an Unrestricted Procurement. The Government must ensure there is adequate competition among the potential pool of available contractors using FBO website. This synopsis is for information and planning purposes only and is not to be construed as a commitment by the Government nor will the Government pays for information solicited. This synopsis is not a solicitation announcement for Request for Quote. No formal solicitation document exists at this time, and no contract will be awarded from this announcement. Respondents will not be notified of the results of the evaluation. All interested contractors should notify this office via email by 12:00 PM (MST) on January 27, 2020. Statement of Work Pharmacy Clean Room Certification General Contractor is to provide all labor, tools, equipment and materials necessary to perform the full scope of work at the Salt Lake City VA Medical Center (SLCVAMC). Contractor employees are to be supervised by a qualified on-site supervisor who will be responsible for employee direction, performance, appearance, conduct, quality of workmanship, safety and OSHA compliance. Contractor must provide a detailed invoice, showing services rendered, date of service, and associated costs. This information serves as evidence of compliance with contract stipulations and is basis for payment. All hazardous materials shall be handled, stored and used in accordance with applicable NFPA, OSHA and EPA standards, as appropriate for each material. The contractor is to preserve and protect all structures, utilities, and equipment on or adjacent to the work site during performance of work required under this contract. Any damage caused by failure to exercise reasonable care shall be the responsibility of the contractor. Pharmacy Clean Room Certification Contractor is to provide certification of three Laminar Flow Work Stations (2 four-foot hoods and one 6-foot hood), one Compounding Aseptic Isolator (CAI), one Biological Safety Cabinet (Chemo Hood), and certification of the clean rooms or segregated compounding area (SCA) in which they reside. The individual performing the tests must be Controlled Environment Testing Association (CETA) trained and certified, and all certifications must be compliant with CETA and CETA Application Guides (CAG) guidelines, as well as USP 800 standards. Includes testing of secondary engineering controls including; Ante rooms x 2, Buffer Rooms x 2, Chemo Storage x 1. Each secondary engineering control area will be certified for all USP 797/800 requirements including but not limited to; Particulate counts, air flow analysis, air exchange rates, room pressure and differential monitoring, viable air sampling, and room smoke test if needed. The primary engineering controls will also be certified and repaired to meet USP 797/800 standards including HEPA filter integrity test, airflow patterns, airflow velocity, particle count, smoke study with video, calibration of hoods, scheduled maintenance and assist with a plan of action for any failure of primary or secondary engineering controls. Scheduled certifications are to be performed at intervals not to exceed 6 months and will be scheduled in advance by the COR. Up to four (4) additional unscheduled certifications may be requested on an as-needed basis. Response to unscheduled requests must be made within 24 hours of notification from COR. Payment shall be made based on work actually performed. Certifications, conclusive reports, and smoke study videos shall be provided to VA for all items. All reports will include all testing and microbial sampling data to demonstrate to outside certifying agencies compliance with USP 797/800. The reports will be made available in electronic format.
