Description
Regional Procurement Office (RPO) - East intends to negotiate on a sole source basis with United Way of Greater Houston, 50 Waugh Drive, Houston, Texas, 77007-5813, to obtain specialist services to identify and refer potentially eligible Veterans to the study team at the Michael E. DeBakey, VAMC, Houston. The services will be used to increase access to VA and community resources for Veterans with diabetes-related distress. The following services will be purchased: ITEM INFORMATION ITEM NUMBER DESCRIPTION OF SUPPLIES/SERVICES QUANTITY UNIT 0001 Contract Base Year: Providing information specialist time for training and identification/referral of potential research participants to MEDVAMC; furthermore, the contractor shall contribute management time to participate in planning for materials development, information specialist training and design of the precise protocols for taking calls and identifying potential research participants. 870 information specialist hours and 87.0 management hours over 12 months Contract Period: Base POP Begin: 02-17-2020 POP End: 02-16-2021 1.00 FE 1001 Option Year 1 870 information specialist hours and 87.0 management hours over 12 months Contract Period: Option 1 POP Begin: 02-17-2021 POP End: 02-16-2022 1.00 FE 2002 Option Year 2 507 information specialist hours and 50.7 management hours over 7 months Contract Period: Option 2 POP Begin: 02-17-2022 POP End: 09-30-2022 1.00 FE Statement of Work (SOW) A. GENERAL INFORMATION Title of Project: IIR HX002580--Using Peer Navigators to increase access to VA and community resources for Veterans with diabetes-related distress. Background: The United Way of Greater Houston and its 2-1-1 Help Line will play an integral role in identification and referral of potentially eligible Veterans to the study team, including Veterans not currently seeking care at the VA who might not otherwise be identified. Over 10,000 Veterans call the help line each year. The help line is staffed by information specialists with training in Veteran benefits and services. Callers are routinely identified by their Veteran status. For this project, Veterans will be asked whether they have diabetes and are interested in a study aimed at helping them to address and cope with it. Names and contact information for those interested will be sent to the study team for recruitment by secure fax or by telephone. Scope of Work: The contractor shall provide information specialist time for training and identification/referral of potential research participants to MEDVAMC; furthermore, the contractor shall contribute management time to participate in planning for materials development, information specialist training and design of the precise protocols for taking calls and identifying potential research participants: Contract Base Year 870 information specialist hours and 87.0 management hours over 12 months � Option Year 1 870 information specialist hours and 87.0 management hours over 12 months Option Year 2 507 information specialist hours and 50.7 management hours over 7 months in Year 3 Contract Total 2,247 information specialist hours and 224.7 management hours Period of Performance: The period of performance (POP) shall be 31 months. The POP will begin with the latter: Start date upon Office of Management and Budget (OMB) approval. Type of Contract: Firm Fixed Price Place of Performance: The contractor shall support this effort at the offices of the United Way of Greater Houston, located at 50 Waugh Drive, Houston, Texas 77007 and any of its call center locations. B. GENERAL REQUIREMENTS Contractor shall conduct business via distance methods (i.e., telephone, email, teleconference) as no budget is allotted for travel for contractors. It is imperative that the contractor be able to provide services within the time frame specified in the Scope of Work as funds available for this project are time-limited. This notice of intent is not a request for competitive proposals. Any responsible sources who wish to identify their interest and capability to provide this equipment or equipment that is compatible must notify this Contracting Specialist at Millicent.Covert@va.gov NLT Monday, February 10, 2020, NLT 11:59 AM EST. Telephone responses will not be accepted. Upon receipt of the interested parties' notification, the Contracting Officer will determine whether to conduct a competitive procurement or proceed with sole source negotiations.
