Page 5 of 5 This request for information (RFI) is issued solely for information and planning purposes only and does not constitute a solicitation. All information received in response to this RFI that is marked as proprietary will be handled accordingly. In accordance with FAR 15.201(e), responses to this notice are not offers and cannot be accepted by the government to form a binding contract. Responders are solely responsible for all expenses associated with responding to this RFI. This notice is published to conduct market research to determine if there is a sufficient number of certified Service-Disabled Veteran Owned Small Businesses (SDVOSB) and/or Veteran Owned Small Businesses (VOSB), capable of performing the requirements to warrant a socio-economic set-aside or full & open competition. It is strongly recommended that interested parties register in the US Department of Veterans Affairs Vet Biz database and the Small Business Administration (SBA) database. Registration may be accomplished at http://www.vip.vetbiz.gov/ and www.sba.gov/, respectively. Interested parties responding to this sources sought request shall submit the following information at a minimum: (1) company name and address, point of contact with phone number, and DUNS number; (2) documentation relating to capability of performance (3) intentions of subcontracting program to include set-asides if any; (4) proof of registration in the VA Vet Biz or SBA websites or intention of registration prior to submission of offers to a solicitation; (5) If you are a distributor a statement from the manufacturer that your company is an authorized distributor; (6) a statement to inform the government if you are a manufacturer or distributor; (7) any information regarding FSS Schedule availability; (8) SAM Registration (9) any other pertinent company documentation. Responses are due by February 24, 2020 at 4:30 pm MST. It is anticipated that a solicitation may be issued in February 2020. If issued, award of a subsequent contract is contemplated in March 2020. Electronic submissions are acceptable via Eric.Carroll@va.gov, and are highly preferred. NO ADDITIONAL INFORMATION IS AVAILABLE OTHER THAN WHAT IS NOTED HEREIN. ALL FIRMS ARE INSTRUCTED NOT TO CALL THE INDIVIDUAL VA MEDICAL CENTERS REQUESTING MORE INFORMATION. This procurement is for continued preventative maintenance and repair services of the da Vinci Surgical Robot. The uptime of the da Vinci Surgical Robot is critical to enabling remote surgery. It is requested that responses include a rough estimate of pricing for completion of the requirements identified below. Purpose: The New Mexico VA Health Care System (NMVAHCS) requires a service agreement to provide continued annual preventive and repair maintenance of the da Vinci Surgical Robot. The uptime of the da Vinci Surgical Robot is critical to enabling remote surgery. Required Services Tools/Equipment/Service Manuals/Documentation The Contractor shall provide all management, tools, parts, supplies, equipment, transportation and labor necessary to maintain and repair the da Vinci Surgical Robot at the New Mexico VA Health Care System (NMVAHCS) in a manner that will ensure continuous and safe operation. This is a full-service contract. All costs associated with the repairs and maintenance services described in this Performance-based Work Statement (PWS) are the sole responsibility of the Contractor, except for any service or replacement parts which the contractor determines are required as a result of the NMVAHCS s failure to use the equipment in accordance with the Contractor s Operator's Manual. The NMVAHCS shall not provide tools, (test) equipment, service manuals, or service diagnostic software to the contractor. The contractor shall obtain, have on file, and make available to its FSE all operational and technical documentation (such as; operational and service manuals, schematics, and parts list) which are necessary to meet the performance requirements of this contract. The location and listing of the service data manuals, by name, and the manuals themselves, shall be provided to the CO upon contract award or request. Service technicians shall be certified on the da Vinci Surgical Robot perform any services/maintenance on the Section 16 specified equipment. Contractor shall provide documentation/copy of service technician certification upon contract award or assignment of a different service technician. Scheduled Maintenance (Preventive Maintenance) The preventive maintenance inspection shall be scheduled in accordance with manufacturer's recommendation and coordinated through Biomedical Engineering. The Contractor shall conduct annual inspections due at 12-month intervals unless otherwise specified by the manufacturer. The annual inspection will be performed at the customer site. The Contractor shall perform preventative maintenance due at 12-month intervals unless otherwise specified by the manufacturer. The contractor shall provide Biomedical Engineering with an itemized list of the recommended PM procedures/checklists indicating work performed and any test/calibration values obtained. Any charges for parts, services, manuals, tools, or software required to successfully complete scheduled PM are included within this contract, and its agreed upon price, unless specifically stated in writing otherwise. The Contractor shall verify that serviced equipment is fully functional before checking out. Following the preventive maintenance inspection, the NMVAHCS will be provided with a completed checklist and a calibration sticker to be placed on the serviced equipment. The Contractor shall supply and install all required updates and upgrades for the da Vinci Surgical Robot. Unscheduled Maintenance (Repair Service) The Contractor shall provide unlimited telephone technical support and onsite service calls. These service calls may be used for any maintenance or repair service as determined by Biomedical Engineering. Response Time: Contractor's FSE or Technical Support must respond by phone to Biomedical Engineering within four (4) hours after receipt of telephoned notification between the hours of 7:30 am to 4:00 pm Mountain Time, Monday through Friday. The Contractor shall furnish at no cost and install all replacement parts in a timely manner to meet uptime requirements. All parts supplied shall be OEM parts compatible with existing equipment. If OEM parts are not available, the Contractor may use Intuitive Surgical approved third-party parts. All parts shall perform identically to the original equipment specifications. The Contractor shall verify that serviced equipment is fully functional before checking out. Compliance with Standards The Contractor shall ensure that the specified equipment functions in conformance with the latest published edition of the applicable standards, such as: NFPA-99, UL, OSHA, VA, CDRH, etc. and the Manufacturer's Performance standards/ specifications. The Contractor shall ensure that any upgrades or updates will meet the required standards/specifications. Deliverables The contractor shall provide service reports within 14 days of completion for all service activities including preventive maintenance, emergency repair, and safety inspections. Each report must, at a minimum, document the following data legibly and in complete detail: Name of contractor and contract number. Name of FSE who performed services. Contractor service number/log number. Date(s) and time period of service. Description of problem reported by Biomedical Engineering Section. Complete description of equipment serviced including device name, model number, serial number, EE number, and any other appropriate identification. Complete description of equipment services performed including scope of work, labor, cause of problems, corrective actions taken, upgrades, and updates. Complete list of all materials, parts, or assemblies installed or replaced including serial numbers of parts where applicable. Total cost to be billed with prior approval from the Contracting Officer, if applicable, for parts not covered or service rendered after normal hours of coverage. Signatures of the FSE who performed the described services and an authorized VA Employee from Biomedical Engineering who witnessed the described services.
